硫化氢对脓毒症大鼠电解质和肾功能的影响

摘要 目的:探讨硫化氢( H2S)对脓毒症大鼠电解质及肾功能的影响。方法92只雄性大鼠,其中20只随机分为正常大鼠组和盲肠结扎穿刺(CLP)6、12、24 h组(每组n=5),72只随机分为假手术组、CLP+生理盐水组(n=18)、CLP+硫氢化钠(NaHS)组(n=18)、CLP+炔丙基甘氨酸(PAG)组(n=18)。术前1 h分别经腹腔注射生理盐水、NaHS和PAG。相应时间点处死后留取血浆标本,使用全自动生化分析仪检测血浆电解质和肌酐( Cr)、尿素氮( BUN)浓度。结果与正常大鼠组比较,CLP术后血浆Na ^+浓度先增高再逐渐降低而后再次升高至正常水平;血浆Cl -浓度逐渐增高至6 h后呈时间依赖性降低;血浆K^+、BUN和Cr浓度均呈时间依赖性升高;术后血浆Ca^2+浓度明显降低。与CLP+生理盐水组比较,CLP+NaHS组术后血浆Na^+、K^+、Cl^-、Cr和BUN浓度均明显降低,Ca^2+浓度变化差异无统计学意义。 CLP+PAG组术后各时间点血浆Na^+、Cl^ -、K ^+、BUN和Cr浓度均明显升高,血浆Ca ^2+浓度变化差异无统计学意义。结论脓毒症大鼠可引起电解质紊乱,早期即出现肾功能损伤并随病程时间延长而逐渐加重。 H2S参与脓毒症时电解质及肾功能调节。脓毒症时H2S可能通过影响肾功能而达到调节血浆电解质浓度的目的。 Objective To investigate the effects of hydrogen sulfide ( H2S ) on serum electrolytes and renal function in sepsis rats .Mtehods A total of 92 male Spargue-Dawley rats were used in the present study.Twenty rats were randomly divided into 5 groups: normal rats group, cecal ligation puncture( CLP) 6 h, 12 h, 24 h group ( n=5/group) .Seventy-two rats randomly divided into sham operate group, CLP+saline group (n=18), CLP+sodium hydrosulfide (NaHS) group (n=18) and CLP +propargylglycine ( PAG ) group ( n=18 ) .Rats were administrated with saline, NaHS or PAG respectively by peritoneal injection 1 h before operation.Rats were sacrificed at designated time and serum was collected to detect serum electrolytes, creatinine ( Cr ) and urea nitrogen ( BUN ) concentrations.Results Compared with normal rats, serum Na^+level increased followed by a decline, then returned to normal level after CLP.Serum Cl-level increased after CLP till 6 h.After 6 h, serum Cl-level started to decline in a time-dependent manner.Serum K^+, Cr and BUN levels increased in a time-dependent manner after CLP.Serum Ca^2+level decreased at all time points after CLP.Compared with CLP+saline group, serum Na^+, Cl^-, K^+, Cr and BUN levels were significantly decreased in CLP+NaHS group.There was no effect on serum Ca^2+level at all time points.At all time points in CLP+PAG group, serum Na^+, Cl^-, K^+, Cr and BUN levels were significantly elevated and serum Ca^2+level do not significantly changed.Conclusion Sepsis induce serum electrolytes disturbance. Renal function is injured at early stage of sepsis and deteriorates in a time-dependent manner.H2 S regulates electrolyte levels and renal function during sepsis and the effects on kidney may involve in regulation of serum electrolyte concentrations during sepsis.
