脓毒症患儿血浆抗凝酶-Ⅲ、D-二聚体与小儿危重评分的关系及其临床意义

摘要 目的:探讨脓毒症患儿血浆抗凝酶(AT)-Ⅲ、D-二聚体(DD)与小儿危重评分(PCIS)的相关性及临床意义。方法:回顾性分析脓毒症患儿(90例)的临床资料。根据PCIS将本组分为非危重组(40例,〉80分)、危重组(28例,71~80分)和极危重组(22例,≤70分);并根据患儿临床结局分为死亡组(21例)和存活组(69例)。分别比较所有患儿入院24 h内血浆AT-Ⅲ活性、DD水平及其与PCIS的相关性。结果:3组AT-Ⅲ活性和DD水平比差异有统计学意义(P〈0.05);脓毒症患儿AT-Ⅲ活性与PCIS评分正相关(P〈0.05);DD水平与PCIS评分负相关(P〈0.05);死亡组AT-Ⅲ活性和PCIS评分明显低于存活组,DD明显高于存活组(P〈0.05)。结论:血浆AT-Ⅲ、DD与其病情危重程度密切相关,可作为判断脓毒症的指标。
