厄洛替尼联合放疗治疗非小细胞肺癌脑转移的临床疗效观察

Clinical Observation of the Treatment of Brain Metastasis of Non Small Cell Lung Cancer With the Combination of Special Therapy and Radiotherapy

目的探讨厄洛替尼联合放疗治疗非小细胞肺癌脑转移的临床疗效。方法以我院收治的34例非小细胞肺癌脑转移患者为研究对象,根据其临床资料进行回顾性分析和总结。患者均接受全脑放疗(40Gy/20次/4周)治疗,同时给予特罗凯(厄洛替尼)150 mg口服,每天1次,连续治疗1个月。在放疗结束后3个月复查脑MRI,观察其病情变化情况,并统计其生存情况。结果患者治疗有效率为76.5%,疾病控制率为100%;中位无进展生存时间为9.5个月,中位总生存时间为11个月;1年生存率为82.4%,2年生存率55.9%;不良反应轻微(7例乏力,5例腹泻,6例皮疹)。结论厄洛替尼联合放疗治疗非小细胞肺癌脑转移的临床效果显著,可提高治疗效果,毒副反应轻,患者可耐受,能延长生存期,具有较高的应用价值。

Objective To investigate the clinical effect of tarceva combined treatment with radiotherapy of brain metastasis of NSCLC. Methods According to the clinical data, it was analyzed and summarized retrospectively, 34 cases of brain metastasis from non small cell lung cancer to our hospital were studied. Patients received whole brain radiotherapy （40 Gy/20 times for 4 weeks） treatment, while giving the Tarceva （erlotinib 150 mg orally, once a day, continuous treatment for a month. MRI was reviewed for 3 months after radiotherapy, and the changes of the disease were observed, and the survival was statistically observed. Results Treatment of patients with effective rate was 76.5% and the disease control rate was 100%, the median progression free survival time was 9.5 months, median survival time was 11 months, 1 year survival rate was 82.4%, 2 year survival rate was 55.9%, adverse reaction of mild （weakness of 7 cases, 5 cases of diarrhea, 6 cases of rash）. Conclusion Special Tarceva combined with radiotherapy for non small cell lung cancer with brain metastasis has a significant clinical effect, improve the therapeutic effect and adverse reaction of light, the patient can be tolerated, can prolong the survival period, has higher application value.
