河南省成人艾滋病阳性家庭流行病学特征分析

Epidemiological characteristics of families with HIV-infected adults in Henan province

目的 了解河南省成人艾滋病阳性家庭流行病学特征及家庭内成人HIV阳性者的免疫状况。方法通过“河南省艾滋病感染家庭随访管理分析系统”和“河南省艾滋病防治数据综合分析系统”收集河南省艾滋病阳性成人家庭相关资料,对其流行病学特征和HIV阳性者的免疫状况进行分析。结果 2011年,河南省有艾滋病成人阳性家庭25 188个,其中HIV阳性人员29 812人;艾滋病成人阳性家庭主要分布在豫东南地区,家庭内HIV阳性人员以35～54岁组为主,占71.37%,83.90%（25 013/29 812）的HIV阳性者接受（过）抗病毒治疗,经血传播者在治构成比为84.82%（19 775/23 313）,高于其他传播途径（χ2=1 692.920,P=0.000）;有72.36%（21 573人）的人HIV阳性者进行了CD4计数检测（CD4M：393 cells/μL,IQR：238～506 cells/μL）,47.97%（14 302人）的人进行了病毒载量检测（VL）;按照家庭类型和传播途径不同对家庭内HIV阳性者CD4计数和VL结果分析,经性途径传播HIV阳性人员HIV病毒量较高,治疗率较低。结论 河南省艾滋病阳性家庭规模大,HIV阳性人员以壮年居多。经性途径传播HIV阳性人员是治疗、救助和帮扶的重点人群。

Objective To study epidemiological characteristics of families with human immunodeficiency virus （HIV）-infected adult members and immune status of the HIV-infected adults in Henan province. Methods Data on families with HIV-infected adult members were collected from AIDS-Infected Family Follow-up Management and Analy- sis System and Henan Province AIDS Data Analysis System and analyzed;the immune status of the adult HIV-infected members in the families was examined. Results There were 25 188 families with HIV-infected adults and 29 812 HIV- infected adults in the families in Henan province in 2011. The families with HIV-infected adults mainly distributed in the southeast area of Henan province. For the HIV-infected adults in the families,71.37% were at age of 35 - 54 years; 83.90 % （25 013/29 812） received antiretroviral therapy ;84. 82% （19 775/23 313 ） were infected through blood transmis- sion, with a significant higher ratio than that infected through other routes （X2 = 1 692. 920, P = 0. 000）. Of the HIV-infec- ted adults ,72. 36% （21 573 ）had CD4 T lymphocyte count,with a mean of 393 cells/p,1 and an interquartile range of 238 -506 cells/μl;47.97% （14 302）had viral load test and those infected through sexual transmission had a higher viral load and a lower rate of antiretroviral therapy. Conclusion The number of families with adult HIV-infected members is large and.most of the HIV-infected members are at the age of 35 - 54 years. The HIV-infected adults with sex transmis- sion are a key population for treatment and aid.
