机车乘务员高血压综合干预效果分析

Effect of comprehensive intervention on hypertension of locomotive crew members

目的对某机务段1 306名机车乘务员进行高血压综合干预与效果分析。方法构建乘务员高血压干预网络体系,对某机务段乘务员进行高血压综合干预,跟踪调查,资料收集与统计分析。结果进行高血压综合干预1年后,1 306名机车乘务员干预后检出高血压358名,高血压患病率27.41%,较干预前新增18例,高血压年罹患率1.38%;高血压知晓、治疗、控制率分别为78.77%、37.15%、32.68%,较干预前明显上升(均P<0.01)。干预后1 306名乘务员群体收缩压、总胆固醇、甘油三酯分别由干预前为(125.58±15.41)mm Hg、(5.30±1.19)和(2.55±2.42)mmol/L下降至(124.07±15.90)mm Hg、(4.87±0.98)和(2.08±2.07)mmol/L(均P<0.05)。340例高血压病患者总体收缩压、舒张压、总胆固醇分别由干预前的(144.09±13.15)和(95.85±8.29)mm Hg、(5.56±1.33)mmol/L下降至(139.59±15.46)和(92.54±10.13)mm Hg、(5.14±0.95)mmol/L(均P<0.05)。高血压病患者及乘务员群体高血压"知、信、行"水平明显上升(均P<0.05)。结论对乘务员采取高血压综合干预措施,可有效提高高血压"知、信、行"水平,控制高血压及并发症风险,降低高血压相关因素水平,收到了良好的高血压综合防治效果。

[Objective]To implement comprehensive intervention members. [Methods]The hypertension intervention network was on hypertension and effect analysis of 1 306 locomotive crew established. Comprehensive intervention, tracking survey, data collection and statistical analysis was performed. [Results]After intervention for 1 years, 358 out of 1 306 locomotive crew members were detected hypertension, with the incidence rate of 27.41%, increasing by 18 cases after intervention. The annual attack rate of hypertension was 1.38%. The rates of knowledge, treatment, control of hypertension was 78.77%, 37.15%, 32.68%, significantly higher after intervention （all P〈0.01 ）. The systolic blood pressure, total cholesterol, triglyceride levels of the 1 306 erew member decreased from（125.58±15.41 ） mmHg,（5.30±1.19） mmol/L and （2.55±2.42） mmol/L to（124.07±15.90） mm Hg, （4.870.98） mmol/L and（2.08±2.07） mmol/L（all P〈0.05）. The systolie blood pressure, diastolic blood pressure, total cholesterol levels of 340 hypertension patients decreased from （ 144.09± 13.15 ） mm Hg, （95.85 ±8.29） mm Hg and （5.56± 1.33 ） mmol/L to （139.59±15.46） mm Hg, （92.54±10.13） mm Hg and（5.14±0.95） mmol/L（all P〈0.05）. The level of ＂knowledge, attitudes and practice＂ of hypertension patients and crew members were significantly improved. [Conclusion]Comprehensive intervention on hypertension of locomotive crew members ean improve the level of ＂knowledge, attitudes and practice＂ of hypertension, eontrol hypertension and eomplication risk, reduce the level of hypertension related factors, and has achieved good effect of hypertension control.
