摘要
目的观察免疫三氧自血疗法治疗寻常型银屑病的疗效及对外周血IL-17/IL-23水平的影响,进一步阐明该方法的作用机制。方法 30例银屑病患者采用免疫三氧自血疗法治疗,治疗结束后4周以银屑病皮损面积和严重程度(PASI)评分评价疗效;采用皮肤CT检测银屑病患者治疗前、后皮损变化情况;同时采用ELISA法检测正常对照组及患者治疗前、后血清IL-17、IL-23的含量,并对其临床疗效进行评价。结果银屑病患者外周血清中IL-17、IL-23水平均显著高于正常对照组,差异均有统计学意义(P<0.01);免疫三氧自血疗法治疗后IL-17、IL-23水平明显下降,与治疗前比较差异有统计学意义(P<0.01)。治疗后PASI评分明显下降,未见明显不良反应,且治疗后皮损通过皮肤CT检测显示皮损的厚度较前变薄,皮损处毛细血管迂曲扩张减轻及血管腔减少。总有效率达83.3%。结论免疫三氧自血疗法治疗银屑病疗效明显;可通过调节患者外周血IL-17/IL-23而发挥治疗作用,值得临床推广。
Objective To observe the curative effect of immune three oxygen self blood therapy in the treatment of psoriasis vulgaris and the impact on peripheral blood IL-17/IL-23 levels,to further explain the mechanism of this method.Method 30 cases of patients with psoriasis were treated by immune three oxygen self blood therapy methods,the therapy effects was assessed according to the psoriasis area and severity index(PASI)score of patients with skin psoriasis after 4 weeks treatment;changes of patients lesions were detected by CT;at the same time,ELISA method was used to detect the content of serum IL-17 and IL-23 before and after treatment in the normal control group,and to evaluate its clinical curative effect.Results IL-17 and IL-23 in peripheral serum of patients with psoriasis were significantly higher than those in the normal control group(P〈0.01);IL-17 and IL-23 decreased significantly after treated by three oxygen self blood method,there was significant difference compared with before treatment(P〈0.01).PASI score decreased significantly after treatment,no obvious adverse reaction.The lesions showed skin thickness became thinner than the previous skin lesions by CT detection,the capillary dilation and vascular lumen were reduced,total effective rate was 83.3%.Conclusion There is a curative effect of immune three oxygen self blood therapy on the psoriasis vulgaris,it can play a therapeutic role through the regulation of IL-17/IL-23 in peripheral blood of patients,which is worth to promotion in clinical.
