健脾清化方联合补救三联治疗耐药幽门螺杆菌感染性胃炎脾虚湿热证的临床研究

Clinical study on Jianpi Qinghua Prescription combined with remedial method for the treatment of drugresistance helicobacter pylori-infected gastritis with the syndrome of spleen deficiency and damp-heat

目的观察健脾清化方治疗耐药幽门螺杆菌(Hp)感染性胃炎脾虚湿热证的临床疗效,以及健脾清化方联合西药补救三联对耐药Hp的根除作用。方法采用随机对照研究,将80例耐药Hp感染性胃炎患者按随机数字表法分为中药组和西药组各40例,中药组予健脾清化方联合补救三联治疗,西药组予标准补救治疗方案治疗,疗程4周。观察2组证候积分、总体疗效以及Hp感染清除率和根除率。结果 2组证候总积分治疗前后比较差异均有统计学意义(P<0.01),治疗后组间差异均具有统计学意义(P<0.01)。2组治疗后组间各证状积分比较,用药4周后胃脘痞胀、主症其他症状、胃脘灼热、恶心呕吐、大便黏滞、伴随其他症状,中药组疗效明显优于西药组(P<0.05)。中药组总有效率94.74%,高于西药组81.08%。中药组耐药Hp清除率为86.84%,根除率为73.68%;西药组对耐药Hp清除率为75.68%,根除率为51.35%。结论健脾清化方联合补救三联对脾虚湿热证耐药HP感染性胃炎有较好的疗效,对根除耐药Hp具有良好的作用。

Objective To observe the clinical curative effect of Jianpi Qinghua Prescription on treating drug-resistance helicobacter pylori（ Hp）- infected gastritis and the eradication of drug-resistance HP with this prescription combined with remedial method. of Methods 80 patients with drug-resistance Hp- infected gastritis were randomly divided into the herbal medicine group and western medicine group of 40 in each group according to the random number table. The treatment group received the Jianpi Qinghua Prescription with remedial method and the control group received the standard remedy. The course of treatment was 4 weeks. the syndrome scores,the overall effect as well as HP infection clearance rate and eradication rate of two groups were observed. Results The difference in total scores of the two groups before and after the treatment had statistic significance（ P〈0. 01）,and the difference between the two groups had statistic significance（ P〈0. 01）. To compare the scores of symptoms between the two groups after the treatment for four weeks,such as fullness and distension in the gastric region and other major ones,burning and hot sensation in the gastric region,nausea,vomiting,sticky and incomplete defecation,and other accompanying ones,the treatment group had got better effect than the control group（ P〈0. 05） with 94. 74% of the total effective rate,which was higher than that of the control group with 81. 08%. Drugresistance HP clearance rate and eradication rate was 86. 84% and 73. 68% respectively in the treatment group and 75. 68% and51. 35% respectively in the control group. Conclusion Jianpi Qinghua Prescription combined with remedial method has good therapeutic effect on treating drug-resistance HP infected gastritis with the syndrome of spleen deficiency and damp-heat,and also has good action on eradicating drug-resistance Hp.
