裂口扩创加内括约肌切开术联合中药治疗慢性肛裂

Rift debridement and internal sphincterotomy for chronic anal fissure

目的观察裂口原位扩创加内括约肌切开术联合中药治疗慢性肛裂的临床疗效。方法将94例慢性肛裂患者随机分为治疗组和对照组,各47例,治疗组采用裂口扩创加内括约肌切开术,对照组采用开放性肛门右后侧切术治疗,2组术后均给予痔疾洗液坐浴及红油膏敷创,观察2组术后创面疼痛、水肿、出血的变化,以及创面感染、裂开等不良反应,并评价临床疗效。结果治疗组术后第3、10天创面水肿、疼痛等症改善情况优于对照组(P<0.05),创面愈合时间、术后创面不良反应以及术后肛门功能评价方面优于对照组(P<0.05)。结论裂口扩创加内括约肌切开术联合中药可减轻慢性肛裂术后并发症和肛门功能损害,缩短创面愈合时间。

Objective To observe the clinical curative effect of rift debridement and internal sphincterotomy f or chronic anal fissure. Methods The 94 cases of chronic anal fissure patients were randomly divided into treat group and control one,respectively 47 cases,and changes of pain,edema,hemorrhage in two groups,adverse reactions such as wound infection,rupture,evaluation of anal function,clinical curative effect. Results Postoperative complication of edema,pain,bleeding in treat group was less than control one at 3 d,10 d（ P〈0. 05）. postoperative anal fuction of treat group was superior to control one（ P〈0. 05）. Conclusion Rift debridement and internal sphincterotomy is effective for chronic anal fissure in relieving complications and anus function damage.
