308nm准分子激光局部照射治疗儿童白癜风63例疗效观察

Local irradiation treatment of vitiligo by 308-nm excimer Laser in 63 children

目的探讨308nm准分子激光局部照射治疗儿童白癜风的疗效及安全性。方法白癜风患儿63例(皮损188处),依据皮损部位分为面颈部组28例(皮损79处),躯干组16例(皮损56处),四肢组13例(皮损37处),肢端组6例(皮损16处),各组均应用308nm准分子激光对皮损进行局部照射,2次/周,间隔时间>72h,12次为1个疗程。2个疗程后评定治疗效果,并进行比较。结果 3处皮损治疗8次痊愈,8处皮损治疗16次痊愈,余均完成2个疗程治疗,总有效率为92.55%(174/188);面颈部组、躯干组、四肢组、肢端组皮损治疗总有效率分别为97.47%、94.64%、86.49%、75.00%,面颈部组和躯干组治疗有效率高于四肢组和肢端组(P<0.05)。结论 308nm准分子激光治疗儿童白癜风疗效满意,尤其是对紫外线敏感皮损。

Objective To observe the clinical efficacy and security of 308-nm excimer laser in the treatment of child vitiligo. Methods Sixty-three children with 188 vitiligo patches were divided into face and neck group （28 patients, 79 vitiligo patches）, trunk group （16 patients, 56 vitiligo patches）, limbs group （13 patients, 37 vitiligo patches） and limb- end group （6 patients, 16 vitiligo patches） according to the location of vitiligo patches. Each patche was irradiated with 308-nm excimer, twice a week, with an interval of 〉72 h, for 12 times as one course. After 2 courses of treatment, the therapeutic effect was evaluated and compared among groups. Results Three patches healed after irradiation for 8 times, 8 patches healed after irradiation for 16 times, and the left patches received two courses of treatment, with the total effective rate of 92.55% （174/188）. The effective rates were higher in face and neck group and trunk group （97.47%, 94.64%） than those in limbs group and limb-end group （86.49%, 75.00%） （P〈0.05）. Conclusion The 308-nm excimer laser is effective for vitiligo, especially for those more sensitive to ultraviolet.
