济南某儿童医院儿童肺泡灌洗液呼吸道病毒及其T淋巴细胞亚群检测

Detection of Bronchoalveolar Lavage Fluid Respiratory Virus and T Lymphocyte Subsets Among Children in a Children's Hospital,Jinan City

目的探讨免疫荧光法在下呼吸道病毒感染诊断中的作用,及下呼吸道病毒感染患儿T淋巴细胞亚群变化规律。方法收集2013年1月1日至2014年12月31日本院住院下呼吸道感染832例患儿肺泡灌洗液,用直接免疫荧光法检测其肺泡灌洗液,流式细胞仪检测阳性患儿T淋巴细胞亚群,并进行统计学分析。结果 367例呼吸道病毒检测阳性,其中呼吸道合胞病毒(RSV)和副流感病毒3型(PIV3)感染例数最多,分别为217例(59.13%)、90例(24.52%)。RSV、ADV为冬季下呼吸道感染的主要致病原因,PIV3为夏季主要病原体。患儿CD3+、CD4+T淋巴细胞水平降低,差异均有统计学意义,CD8+T淋巴细胞降低水平差异无统计学意义(P>0.05),与其他病毒感染性疾病表现相似。结论直接免疫荧光法可以快速、准确地检测多种呼吸道病毒抗原,为临床提供诊断依据,具有重要的临床应用价值。下呼吸道病毒感染与其他病毒感染性疾病在T淋巴细胞亚群中的表现相似。

Objective To explore the effectof immunofluorescence in the diagnosis of lower respiratory tract infection and varying pattern T lymphocyte subsets in children with the disease. Methods Bronchoalveolar lavage fluid were collected from 832 children with lower respiratory tract infection in the hospital from January 1,2013 to December 31,2014.Direct immunofluorescence was used to test the bronchoalveolar lavage fluid and T lymphocyte subsets were detected with flow cytometry method.Data from these tests were analyzed by statistical method. Results A total of 367 cases tested were positive for respiratory viruses,and RSV and PIV3 infection were the most,including 217cases（59.13%）and 90cases（24.52%）for each type.RSV and ADV were the major causes of lower respiratory tract infection in winter and PIV3 was the major cause in summer.Level of CD3＋,CD4＋T lymphocytes in children significantly decreased（P〈0.05）while the level of CD8＋T lymphocytes did not significantlydecreased（P〉0.05）,these results were similar to other viral diseases. Conclusion Direct immunofluorescence method can detect multiple respiratory viruses antigen quickly and accurately,which has important clinical value.T lymphocyte subsets are similar in lower respiratory tract infection and other viral diseases.
