慢性阻塞性肺疾病合并肺间质纤维化的临床研究

A clinical analysis of chronic obstructive pulmonary disease with pulmonary fibrosis

目的 探讨慢性阻塞性肺疾病(COPD)合并肺间质纤维化的特点及二者之间的关系。方法 对23例诊断为COPD合并肺间质纤维化患者的病史、体征、X线胸片、高分辨CT(HRCT)、肺功能及血气检查结果进行对比分析。结果 根据临床症状,COPD合并肺间质纤维化患者可分为COPD为主型和肺间质纤维化为主型,所有患者均具有较长吸烟史,肺功能测定多为混合性通气功能障碍,X线胸片及HRCT兼有两种疾病的特点。结论 COPD合并肺间质纤维化使原来单一疾病的特点不典型或两种疾病的特点共存;HRCT在COPD合并肺间质纤维化的诊断中具有非常重要的价值。

Objective To compare the clinical characteristics of and analysis the relationship between the chronic obstructive pulmonary disease (COPD)and interstitial pulmonary fibrosis. Methods 23 patients with COPD and pulmonary fibrosis were analyzed by clinical symptoms,radiographic features, arterial blood-gas analysis, lung function test and computed tomography of chest.Results According to the clinical features patients could be classified into COPD type and pulmonary fibrosis type. Lung function test showed mixed ventilation disturbances. X-rays and CT demonstrated some features of both COPD and pulmonary fibrosis.Conclusions COPD and interstitial pulmonary fibrosis might coexist in the same individual,so the clinical characteristics of each entity overlape and become obscure. HRCT is a valuable tool to distinguish these conditions.
