和胃降逆法对反流性食管炎患者食管黏膜cyclin D_1、CDK_4表达的影响

Effects of Stomach- regulating and Qi- lowering Method(和胃降逆法) on Expressions of cyclin D_1 and CDK_4 in Esophageal Mucosa of Patients with Reflux Esophagitis

目的:通过观察和胃降逆法对反流性食管炎患者食管黏膜cyclin D1(细胞周期蛋白1)、CDK4(细胞周期蛋白依赖性激酶)表达的影响,探讨和胃降逆法治疗反流性食管炎的作用机理。方法:选择120例反流性食管炎患者,分为中药治疗组、西药治疗组。中药治疗组以和胃降逆法治疗,选用和胃降逆方1包;西药治疗组予奥美拉唑胶囊20 mg;两组均1日2次,共8周。两组患者治疗前后均行胃镜检查及食管黏膜病理活检,观察临床疗效及食管黏膜病理学改变,免疫组化法测定食管黏膜cyclin D1、CDK4的表达。结果:和胃降逆法能明显改善反流性食管炎患者的临床症状及组织病理学积分,作用与西药相当;能明显降低反流性食管炎患者食管黏膜组织CDK4、cyclin D1的表达,其CDK4表达治疗后低于西药组,有统计学意义。结论:和胃降逆法能明显降低反流性食管炎患者食管黏膜组织CDK4、cyclin D1的表达,是其治疗反流性食管炎的作用机理之一。

Objective： To observe the effects of stomach- regulating and Qi- lowering method on the expressions of cyclin D1 and CDK4in esophageal mucosa of patients with reflux esophagitis,and explore the effective mechanism of the therapy of stomach- regulating and Qi- lowering in the patients of reflux esophagitis.Methods： 120 patients with reflux esophagitis were divided into two groups： Chinese medicine group and western medicine group. The Chinese medicine group was treated with the therapy of stomach- regulating and Qi- lowering,these cases were given 1 dose of Chinese herb of stomach- regulating and Qi- lowering,while the western medicine group was treated with Omeprazole capsule. The medicine for two groups were taken twice daily for 8weeks. All patients were taken examination of gastroscope and biopsy of esophageal mucosa before and after treatment,to observe the clinical therapeutic effect and esophageal mucosa pathological change of patients with reflux esophagitis,expressions of cyclin D1 and CDK4in esophageal mucosa were assayed by immunohistochemistry. Results： Stomach- regulating and Qi- lowering method could ameliorate the clinic symptoms and the pathologic changes of esophageal mucosa tissue of reflux esophagitis,and significantly decrease the expressions of cyclin D1 and CDK4in esophageal mucosa,the CDK4 expression level in Chinese medicine group was lower than that of western medicine group after treatment. Conclusion： Stomach- regulating and Qi- lowering method could significantly decrease the expressions of cyclin D1 and CDK4in esophageal mucosa of patients with reflux esophagitis,and it may be one of the mechanisms of treating reflux esophagitis.
