达克罗宁胶浆在全麻手术经口气管插管中的应用效果

Dyclonine mortar in the application of the breath tube intubation on general anesthesia surgery

目的探讨达克罗宁胶浆作为润滑剂用于经口气管插管患者中的临床效果。方法选择全麻经口气管插管患者86例,用随机数字表法分为观察组和对照组,每组43例。在麻醉诱导前,观察组采用1%达克罗宁胶浆作润滑剂均匀涂抹于气管导管前15~20 cm,对照组常规应用石蜡油润滑气管插管,采用视觉模拟评分法(visual analog score,VAS)(0~10分)对患者进行咳嗽、咽痛和声嘶症状的评估;采用WHO黏膜反应分级法(0~IV度记录为0~4分,总分为咽部和喉部得分之和)对患者咽部和喉部黏膜反应进行评分。结果术后两组间咳嗽VAS评分比较差异无统计学意义(P>0.05);术后两组患者咽痛、声嘶VAS评分观察组小于对照组,差异有统计学意义(P<0.05);术后两组患者咽喉部黏膜反应评分观察组小于对照组,差异有统计学意义(P<0.05)。结论应用达克罗宁胶浆在气管插管中作为润滑剂,能有效地减少神经冲动刺激咽喉壁,减少患者的痛苦,提高患者的依从性,且操作简单。

Objective To study the clinical effect of dyclonine mortar as a lubricant for the breath tube intubation in critical care patients. Methods According to the random number table method, 86 patients with the breath tube intubation anesthesia were divided into observation group and control group, 43 cases in each group. After anesthesia induction, patients in observation group were with 1% dyclonine mortar as a lubricant evenly 15 ~ 20 cm in front of the endotr^heal tube for endotracheal intubation, and the control group were used routinely paraffin oil lubricating oil endotracheal tube. The visual analogue scale method （visual analog score, VAS） （0 - 10） was used for patients＇ cough, sore throat and hoarseness symptoms assessment; WHO mucosa reaction classification method （0 - IV degree 0 to 4 points, has been recorded in the sum of total score for the pharyngeal and laryngeal score） on patients with pharyngeal and laryngeal mucosa reaction rate. Results Postoperative cough VAS score between the two groups had no statistical difference （P 〉 0.05）, The patients＇ postoperative sore throat, hoarseness VAS score in observation group were less than those in the control group, and the differences were statistically significant （P 〈 0.05 ） ; patients＇ postoperative throat mucosa reaction score of observation group is less than that of the control group, and the difference was statistically significant （P 〈 0.05 ）. Conclusion Application of dyclonine endotracheal intubation as a lubricant, can effectively reduce the impulse stimulation bronchial wall, reduce the pain of patients, improve patient compliance, and the operation is simple, which is worth popularizing in clinic.
