常规静息心电图ST-T改变在老年冠心病诊断中的临床价值

Clinical value of ST-T changes of conventional resting ECG in the diagnosis of coronary heart disease for elderly patients

目的探讨常规静息心电图ST-T改变在老年冠心病诊断中的临床价值。方法分析2012~2014年收治的具有冠心病临床表现的老年患者共87例,行常规静息心电图及冠状动脉造影检查,比较2种检查结果的阳性率,进行分析。结果本组87例经临床症状诊断为冠心病的患者中,其中33例常规静息心电图表现为ST-T改变,阳性率为37.93%,73例经冠脉造影诊断为冠心病,阳性率为83.9%,两组阳性率差异有统计学意义（P〈0.05）。结论常规静息心电图ST-T改变在老年冠心病的诊断中不能作为确诊依据,仅可作为辅助诊断手段,临床确诊需要就临床表现及各项检查进行综合评判。

Objective To investigate the clinical value of ST-T changes of conventional resting ECG in the diagnosis of coronary heart disease for elderly patients. Methods 87 elderly patients with coronary heart disease treated from 2012 to2014 were selected. Through the conventional resting ECG and coronary angiography, the positive rate based on two methods were compared and analyzed.. Results For 87 CHD patients, there were 33 cases with ST-T changes of conventional ECG ; the positive rate was 37.93%; 73 cases were confirmed with coronary heart disease through coronary angiography;the positive rate was 83.9%, the positive rate based on two methods was significantly different （P 〈 0.05）. Conclusion The ST-T changes of conventional resting ECG can＇ t be used as the diagnosis basis in the diagnosis of coronary heart disease for elderly patients and can only be used as an auxiliary means. It requires a comprehensive evaluation on the clinical diagnosis based on the clinical manifestation and examinations.
