乌司他丁联合生长抑素治疗老年急性重症胰腺炎的疗效及对患者血清TNF-α、IL-6、IL-8影响的研究

Effect of ulinastatin combined with somatostatin on acute severe pancreatitis of old patients and their serum TNF-α,IL-6 and IL-8 levels

目的分析乌司他丁联合生长抑素(SS)治疗老年急性重症胰腺炎(SAP)的疗效及对患者血清TNF-α、IL-6、IL-8水平的影响。方法选择在天津市职业病防治院接受住院治疗的老年SAP患者作为研究对象,随机分为接受常规治疗的对照组和接受乌司他丁联合SS治疗的观察组,比较两组患者的临床症状体征恢复时间、血淀粉酶及肝肾功能、血清TNF-α、IL-6、IL-8水平等差异。结果观察组患者接受治疗后的各项临床症状体征恢复时间均短于对照组患者,差异有统计学意义(P<0.05);观察组患者接受治疗后的AMY、ALT、Cr值均显著低于对照组患者,差异有统计学意义(P<0.05);观察组患者接受治疗后的血清TNF-α、IL-6、IL-8值均明显低于对照组患者,差异有统计学意义(P<0.05)。结论乌司他丁联合SS治疗有助于加速SAP患者病情康复,降低患者的血淀粉酶值并改善其肝肾功能、优化血清炎症因子水平,具有积极的临床意义。

【Objective】To analyze the effect of ulinastatin combined with somatostatin on acute severe pancreatitis of old patients（SAP） and their serum TNF-α, IL-6 and IL-8 levels.【Methods】The old patients with acute severe pancreatitis in our hospital were chosen as the research subjects and randomly divided into control group with routine treatment and observation group with ulinastatin combined with somatostatin treatment. The differences in the recovery time of clinical symptoms and signs, blood amylase（AMY）, liver and kidney function, serum TNF-α, IL-6and IL-8 levels were compared between both groups.【Results】The recovery time of symptoms and signs in the observation group was significantly shorter than that of the control group（P 0.05）. The AMY, ALT and Cr values in the observation group were significantly lower than those in the control group（P 0.05）. The serum TNF-α, IL-6and IL-8 values in the observation group were significantly lower than those in the control group（P 0.05）.【Conclusions】Ulinastatin combined with somatostatin treatment helps patients to recover faster, reduce blood amylase value, improve kidney function and optimize serum levels of inflammatory factors with positive clinical significance.
