鼻中隔高位偏曲与慢性鼻-鼻窦炎相关性研究

Correlation study of high deflection of nasal eptum with chronic nasal sinusitis

目的：研究鼻中隔高位偏曲与慢性鼻-鼻窦炎在临床治疗中的相关性，并找到该病的有效治疗方案。方法对100例鼻中隔高位偏曲伴慢性鼻-鼻窦炎的患者进行鼻内镜下鼻窦开放手术及鼻中隔偏曲术治疗，对另外100例患者进行保守治疗。结果全部200例鼻中隔高位偏曲患者中前组鼻窦炎142例，占全部患者的71.0%，鼻中隔高位偏曲的患者患有前组鼻窦炎较后组鼻窦炎高，且差异有统计学意义（P ﹤0.05）；手术患者术后仅有3例患者出现复发的情况，而对照组鼻窦炎复发患者48例；在治疗后的12周以及24周后，实验组 VSA 评分低于对照组，且患者自我感觉也相对较好。结论对患有慢性鼻-鼻窦炎伴有鼻中隔高位偏曲的患者，在临床治疗中采用鼻内镜下鼻窦开放手术及鼻中隔偏曲术治疗，临床治疗效果良好，与保守治疗患者相比复发率明显降低，术后患者头闷痛及鼻腔通气改善明显，对抑制患者鼻窦炎的复发有积极作用。

Objective To study the correlation of high deflection of nasal septum with chronic nasal sinusitis,and find effective treatment. Methods One hundred patients with high deflection of nasal septum complicated with chronic nasal sinusitis were conducted open nasal endoscopic sinus surgery,and another 100 cases were treated conservatively. Results Among all the 200 patients with deflection of nasal septum,142 cases had former group sinusitis,accounting for 71. 0% ,patients with deflection of nasal septum were more likely to suffer former group sinusitis than posterior nasal sinuses,and the difference was significant（ P = 0. 031 ﹤0. 05）. Three cases had postoperative recurrence in patients conducted surgery and 48 patients with recurrent sinusitis in the control group. After treatment for 12 weeks and 24 weeks,the VSA score in experiment group was lower than that of the control group,and the patients felt better. Conclusions For patients with chronic nasal sinusitis complicated with high deflection of nasal septum,open surgery in the clinical treatment with nasal endoscopic sinus surgery is effective,and with lower recurrence rate in comparison with patients received conservative treatment;head stuffy pain and nasal ventilation improved obviously,can effectively inhibit with recurrence.
