27HC代谢酶与乳腺癌预后因素相关性研究

摘要 目的:探讨乳腺癌患者血浆胆固醇27-羟化酶(CYP27A1)和胆固醇7α羟化酶(CYP7B1)浓度与乳腺癌临床分期、组织学分级、Ki-67等预后因素的相关性。方法:观察分析了34例乳腺癌患者血浆CYP27A1和CYP7B1浓度,及其与血清总胆固醇、乳腺癌临床分期、组织学分级、Ki-67等指标的关系。结果:34例乳腺癌患者中,总胆固醇升高者16例,总胆固醇水平正常者18例,血清总胆固醇升高的乳腺癌患者,其血浆CYP27A1酶浓度较总胆固醇正常患者高(P<0.05),而CYP7B1酶浓度明显低于总胆固醇正常患者(P<0.01)。Ki-67高表达乳腺癌患者血中27HC合成酶CYP27A1水平较Ki-67低表达者高(P<0.05);而27HC降解酶CYP7B1水平与Ki-67水平的关系则正好相反。临床Ⅰ期乳腺癌患者血浆CYP27A1浓度明显低于Ⅱ、Ⅲ、Ⅳ期,与之相反,CYP7B1浓度却高于Ⅱ、Ⅲ、Ⅳ期患者,结果均有显著性差异(P<0.05);Ⅱ、Ⅲ、Ⅳ期乳腺癌患者间血浆CYP27A1、CYP7B1浓度无显著性差异(P>0.05)。组织学分级间亦有类似的结果。结论:Ki-67低表达、分化好的早期乳腺癌患者,其血浆CYP27A1水平偏低,CYP7B1水平较高,27HC代谢酶与乳腺癌预后因素具有一定的相关。 Objective:To investigate the relationship between plasma concentrations of CYP27A1, CYPTB1 in patients with breast cancer and clinical stages, pathological grade and Ki-67 prognosis factors. Methods:The relationship between plasma concentrations of CYP27A1, CYPTB1 and total cholesterol, clinical stages, pathological grade and Ki-67 of breast cancer among 34 patients with breast cancer were observed. Results:In the 34 patients with breast cancer, 16 cases had elevated total cholesterol, 18 cases had normal total cholesterol. In the patients who had elevated total cholesterol, the concentration of CYP27A1 was higher than its in patients who had normal total cholesterol ( P〈0.05 ), and the concentration of CYP7B1 was lower than its in patients who had normal total, cholesterol ( P〈0.01 ). The level of CYP27A1 synthase of 27HC in breast carcinoma patients with high expression of Ki-67 was higher than its in patients with low expression of Ki-67. But the relationship between the level of CYP7B1 degrading of 27HC and Ki-67 level was just the opposite. The plasma concentration of CYP27A1 in breast carcinoma patients with Phase I clinical stage was significantly less than its in patients with Phase II, III, IV clinical stages. Instead, the plasma concentration of CYP7B 1 was higher than its in patients with Phase II, III, IV clinical stages, the differenceS were statistically significant ( P〈0.05 ). The concentrations of CYP27A1, CYPTB1 among patients with Phase II, III, IV clinical stages showed no significant difference ( P〉O.05 ). And the analytical results were also similar in the pathological grade of patients with breast cancer. Conclusion : In the patients with low expression and well-differentiation of Ki-67 early breast cancer, the plasma CYP27A 1 level is low, and the CYP7B1 level is high. The 27HC metabolic enzyme and the prognosis of breast cancer are relevant.
