沙美特罗替卡松粉吸入剂与氨茶碱片治疗女性慢性阻塞性肺疾病稳定期的疗效比较

Comparison of salmeterol fluticasone propionate powder for inhalation and aminophylline tablets treating female COPD with remissionand

目的比较沙美特罗替卡松粉吸入剂与氨茶碱片治疗女性缓解期慢性阻塞性肺疾病(COPD)的效果及其对IL-6、IL-8的影响。方法选取2012年1月至2014年6月川北医学院附属医院收治的女性缓解期COPD患者160例,按照随机数字表法随机分为观察组和对照组各80例。观察组吸入沙美特罗替卡松粉吸入剂,对照组口服氨茶碱片进行治疗。比较2组治疗后的效果、深吸气量(IC)、1s用力呼气容积(FEV1)、用力肺活量(FVC)、第一秒用力呼气容积占预计值的百分比(FEV1%)等肺功能指标及白细胞介素(IL)-6、IL-8水平。结果观察组总有效率87.5%高于对照组72.5%,差异有统计学意义(P<0.05);观察组在治疗后IC、FEV1、FVC、FEV1%等指标升高明显,呼吸困难指数(TDI)、6min步行距离(6MWD)也更高,差异有统计学意义(P<0.05);2组在治疗前IL-6、IL-8水平比较差异无统计学差异(P>0.05),2组治疗后IL-6、IL-8水平均较治疗前降低,但治疗后观察组IL-6、IL-8水平下降更明显,差异有统计学意义(P<0.05)。结论沙美特罗替卡松粉吸入剂与氨茶碱片治疗女性缓解期COPD均呈现高效结果,但相对来说沙美特罗替卡松吸入治疗效果更佳,对呼吸困难缓解程度更好。

Objective To compare the efficacy of Salmeterol fluticasone propionate inhalation and Aminophyl‐line Tablets treating female chronic obstructive pulmonary disease（COPD） with remission and the influence on IL‐6 , IL‐8 levels .Methods During January 2012 to June 2014 ,160 cases of female remission of COPD patients were select‐ed and were divided into observation group and control group with 80 cases in each group according to digital meth‐od .The observation group inhaled salmeterol fluticasone propionate powder for inhalation ,while the control group o‐ral aminophylline tablets for treatment .The curative effect and IC ,FEV1 ,FVC ,FEV1% lung function index and IL‐6 ,IL‐8 levels were compared between the two groups .Results Observation group total effective rate 87 .5% was higher than the control group 72 .5% ,the difference was statistically significant（P 〈 0 .05） .Observation group IC , FEV1 and FVC ,FEV1% index after treatment increased obviously ,TDI ,6 MWDS is also higher ,the difference was statistically significant（P〈0 .05） .In the two groups before treatment IL‐6 ,IL‐8 levelsare of no statistical difference （P〉0 .05） ,the two groups after treatment IL‐6 ,IL‐8 levelswere reduced ,the observation group after treatment IL‐6 , IL‐8 levels drop were more obvious ,the difference was statistically significant（P〈 0 .05） .Conclusion Salmeterol flu‐ticasone propionate inhalation and aminophylline tablets in the treatment of female remission of COPD showed a high efficient results ,but the relative effect of salmeterol fluticasone propionate inhalation therapy effect is better ,the de‐gree of dyspnea alleviated better .
