彩色多普勒超声联合尿蛋白肌酐对高血压性早期肾损害的诊断价值

Value of color Doppler ultrasound combined with urinary protein/creatinine on the diagnosis of early renal injury in hypertension

目的探讨彩色多普勒超声联合尿蛋白肌酐检测技术对高血压性早期肾损害患者的诊断价值。方法选取本院2012年2~8月的68例高血压性早期肾损害患者作为研究对象,均给予厄贝沙坦片和拜阿司匹林片治疗,并加用自拟＂安泰固肾汤＂和保肾康片,连续治疗1个月。所有受检者均进行尿蛋白/肌酐（Um Alb/UCr）及彩色多普勒超声检查。随访3个月后,比较彩色多普勒超声联合Um Alb/UCr检测与单纯尿蛋白肌酐检测两种检查方法的阳性率、准确度、灵敏度及特异度等。结果 Um Alb/UCr联合彩色多普勒检查的阳性率、准确度、灵敏度、特异度显著高于单纯性Um Alb/UCr检查,差异有统计学意义（P〈0.05）。结论彩色多普勒超声联合尿蛋白肌酐检查检测技术对高血压性早期肾损害患者具有重的诊断价值,值得临床推广应用。

Objective To explore the diagnostic value of color Doppler ultrasound combined with urinary protein/creatinine examination on patients with early renal injury in hypertension. Methods 68 cases with early renal injury in hypertension from February 2012 to August in our hospital were selected and given irbesartan tablets and aspirin tablets,Antai Gushen Decoction and Baoshenkang tablets were added,continuous treatment for 1 months.Urinary protein/creatinine （UmAlb/UCr）,Doppler ultrasound examination were given to all the patients.After 3 months of follow-up,the positive rate,accuracy,sensitivity and specificity of UmAlb/UCr combined with Doppler ultrasound and UmAlb/UCr only was compared. Results The positive rate,accuracy,sensitivity and specificity of UmAlb/UCr combined with Doppler ultrasound examination was higher than that of UmAlb/UCr examination only,with significant difference（P〈0.05）. Conclusion Color Doppler ultrasound combined with urine protein/creatinine detection technology has important diagnostic value on patients with early renal injury in hypertension,it is worthy of clinical promotion and application.
