B型尿钠肽在急性冠脉综合征预后判定中的应用价值

The application value of B type natriuretic peptide in the prognosis judgement of acute coronary syndrome

目的探讨B型尿钠肽（BNP）在判定急性冠状动脉综合征患者预后情况中的临床预测价值。方法选取本院2011年3月~2013年12月收治的125例急性冠状动脉综合征患者作为研究对象,根据症状类型分为急性ST抬高型心肌梗死组（Ⅰ组）42例、非ST抬高型心肌梗死组（Ⅱ组）43例、不稳定型心肌梗死组（Ⅲ组）40例。另选取健康者40例作为对照组。所有患者入院后测定血浆BNP浓度,并行超声心动图检查。结果Ⅰ组、Ⅱ组的BNP水平显著高于对照组,差异有统计学意义（P〈0.05）。Ⅰ组的BNP水平显著高于Ⅱ组,Ⅱ组的BNP水平显著高于Ⅲ组,差异有统计学意义（P〈0.05）。Ⅰ组、Ⅱ组的LVEF水平显著低于正常水平及对照组,差异有统计学意义（P〈0.05）。Ⅰ组的LVEF水平显著低于Ⅱ组和Ⅲ组,Ⅱ组的LVEF水平显著低于Ⅲ组,差异有统计学意义（P〈0.05）。结论 BNP水平可以预测急性冠状动脉综合征患者的预后情况,不同类型急性冠脉综合征患者的血浆BNP水平不同,有助于早期诊断及预后判定。

Objective To explore the clinical predictive value of BNP in judging the situation of patients with acute coronary syndrome. Methods 125 patients with acute coronary syndrome from March 2011 to December 2013 in our hospital were selected as the research objects.According to the type of syndrome,all patients were divided into group Ⅰ of 42 cases,GroupII of 43 cases and GroupⅢ of 40 cases,and selected the other healthy people of 40 cases as control group.The level of plasma BNP concentration and ultrasonic Heartbeat graph examination were measured after admis-sion. Results The level of plasma BNP concentration in group Ⅰ and group II was higher than that in the control group,with significant difference （P〈0.05）.The level of plasma BNP concentration in group Ⅰ was higher than that in groupII,with significant difference（P〈0.05）.The level of plasma BNP concentration in groupII was higher than that in groupⅢ,with significant difference（P〈0.05）.The level of LVEF in group Ⅰ and groupII was lower than that the normal level and in the control group,with significant difference （P〈0.05）. The level of LVEF in group Ⅰ was lower than that groupII and groupⅢ,with significant difference（P〈0.05）.The level of LVEF in groupII was higher than that in groupⅢ,with significant difference （P〈0.05）. Conclusion The level of BNP can predict the prognosis of patients with acute coronary syndrome,different types of acute coronary syndrome patients with different levels of plasma BNP,which con-tribute to early diagnosis and prognosis judgment.
