自制尿道导向器在首次导尿失败中的应用

Application of self-made urethral guide in the first urethral catheterization failure

目的:探讨自制尿道导向器在首次导尿失败中的应用和疗效。方法:将56例首次导尿失败的患者随机分成两组,一组28例,采用自制尿道导向器导尿(自制组),保持导向器头部轻贴尿道12点缓慢推进;一组28例,采用传统的补救方法,如导尿管内置金属导丝、尿道扩张、手法协助尿管等(对照组)。结果:自制组28例患者均一次操作成功,顺利置入导尿管,无明显尿道疼痛和血尿;对照组28例患者导尿成功20例,失败8例,其中伴有尿道疼痛11例,肉眼血尿8例。两组疗效比较,差异有统计学意义(P<0.05)。结论:自制尿道导向器在首次导尿失败中应用效果良好,操作快速简单,能确保导尿成功,有显著优势。

Objective： To discuss the application of self-made urethral guide in the first urethral catheterization failure and its curative effect. Method： Fifty-six cases with the first urethral catheterization failure were divided into two groups. Self-made urethral guide was used in one group （28 patients）. We slowly moved it along 12 points of urethra then reached the bladder. Another group （28 patients） experienced the traditional methods such as metal guidewire, urethral dilatation, manual assistance on the urinary catheter. Result： The 28 cases using self-made urethral guide were all successful, and no urethral pain or hematuria was found. Only 20 patients using the traditional methods were successful. Moreover, 11 of them experienced urethral pain and eight of them suffered from macroscopic hematuria. Comparison of two groups was significantly different （P〈0.05）. Conclusion： The application effect of self-made urethral guide in the first urethral catheterization failure is good, which could ensure the success of the urethral catheterization.
