摘要 全机器人不停跳冠状动脉旁路移植术创伤小,术后恢复快,是未来微创冠状动脉外科的趋势,对麻醉管理提出更高的要求,主要包括加压CO2气胸、单肺通气的应用、经食道超声心动图的监测、术中血流动力学的维持、体外循环及中转开胸手术的准备、室颤的处理等,要求麻醉医师对患者病理生理的调控及围术期的管理达到更精细化。 Robotic totally endoscopic coronary artery bypass on beating heart (BH-TECAB), a minimally invasive surgical approach with rapid postoperative recovery, is becoming the future trend of minimally invasive coronary surgery, which demands a higher standard for anesthetic management. It comprises pressured pneumothorax, unilateral lung ventilation, transesophageal echocardiography monitoring, hemodynamics maintaining, preparation for cardiopulmonary bypass and conversion to sternotomy, and ventricular fibrillation treatment, which require anesthesiologists to achieve more refined perioperative management and pathophysiological regulation.
