摘要
目的观察不同分型肠道病毒71型(EV71)感染手足口病患儿血清内皮素-1(ET-1)水平的变化与疾病预后间的关系,为临床治疗提供参考依据。方法选取2011年1月-2014年1月医院收治的EV71感染手足口病患儿130例,根据患儿临床特征,分为普通组41例、重症组64例与危重症组25例,对比3组患儿心率、血白细胞计数、血糖、ET-1等水平,数据采用SPSS 18.0软件进行统计分析。结果重症组与危重症组患儿心率、血白细胞计数均较普通组显著升高(P<0.05),危重症组较普通组血糖水平明显升高(P<0.05),与重症组相比较,危重症组年龄偏小、血糖水平偏高;ET-1水平危重症组患儿明显高于普通组与重症组,差异均有统计学意义(P<0.05);多元logistic回归分析发现,ET-1水平是导致危重症患儿不良事件发生的独立危险因素(P<0.05)。结论ET-1水平是导致EV71患儿危重症以及不良事件发生的独立危险因素,临床检测ET-1水平,对EV71患儿临床治疗与预后有着重要的意义。
OBJECTIVE To observe the changes of serum ET‐1 level of children with HFMD infected by different types of EV71 and its relationship with prognosis of the disease ,so as to discuss the risk factors of progress of EV71 .METHODS Totally 130 children infected by EV71 were clinically chosen and according to clinical character‐istics divided into normal group (41 cases) ,severe group (64 cases) and critical group (25 cases) .Heart rates , white blood cell numbers ,blood sugar level and ET‐1 level of these children in the three groups were detected and analyzed statistically by SPSS 18 .0 software .RESULTS Heart rates and white blood cell numbers of severe group and critical group both improved obviously compared with the normal group (P〈0 .05) .Blood sugar level of the critical group was higher than that of the normal group (P〈0 .05) .Compared with the severe group ,age of the critical group was smaller and blood sugar level was higher .On ET‐1 level ,the critical group was significently higher than that of the normal group and the severe group (P〈0 .05) .Multiple logistic regression analysis showed that ET‐1 was an independent risk factor causing adverse events of children with severe and critical symp‐toms (P〈0 .05) .CONCLUSION ET‐1 level was an independent risk factor causing adverse events of children with severe and critical symptoms .Clinical detection of ET‐1 level has significances for the treatment and prognosis of children infected by EV71 .
