隆突性皮肤纤维肉瘤的临床及影像学表现

Clinical and imaging manifestations of dermatofibrosarcoma protuberans

目的探讨隆突性皮肤纤维肉瘤的临床及影像学表现,提高对本病的认识及诊断准确性。方法回顾性分析31例经手术病理证实的隆突性皮肤纤维肉瘤病例的临床及影像学资料。结果 31例均发现皮肤结节或肿块就诊,1例右上肢疼痛,1例左下肢疼痛,余29例无明显症状;单发12例,多发19例;首发15例,复发16例;躯干20例,四肢8例,颌面部2例,头皮1例;合并肺转移5例;密度均匀25例,密度不均匀6例,边界清晰23例,周围脂肪间隙模糊6例,侵犯横纹肌10例;增强扫描12例,实性部分均明显强化。结论隆突性皮肤纤维肉瘤具有一定的临床及影像学表现特点,综合分析,对肿瘤的术前诊断及疗效评估具有重要价值。

Objective To investigate the clinical and imaging manifestations of dermatofibrosarcoma protuberans and to im-prove the understanding and diagnostic accuracy of this disease. Methods Clinical and imaging datas of 31 cases of dermatofi-brosarcoma protuberans were analsiyzed retrospectively. Results Of 31 cases,skin noudles or masses were found initially,1 case of right upper limb pain,1 case of left lower extremity pain,the others no symptoms;12 cases of single,19 multiple;15 firstly,16 palindromically;20 in trunk,8 extremities,2 maxillofaciall region,1 scalp;5 cases with pulmonary metastasis;25 homogeneous in density,6 inhomogeneous;23 with definded margin,6 peripheral fat gap unclear,2 muscle invasion;the solid part of tumors en-chanced obviously of 12 cases. Conclusion Comprihensive analysis of certain clinical and imaging characteristics of dermatofi-brosarcoma protuberans is important on the peri-operative diagnosis and post-operative evaluation.
