胰腺脂肪替代2例并文献复习 被引量:1

摘要 病例资料病例1,男,74岁,因中上腹隐痛1月余入院。30年前因胆囊结石行胆囊切除术。入院后空腹血糖7.29 mmol/L,红细胞4.83x1012/L,血甘油三酯0.73 mmol/L,血脂肪酶为25 U/L,血淀粉酶25.7 U/L。超声检查:胰腺形态、大小未见异常,实质回声增强,主胰管未见扩张。CT 检查:肝脏密度低于同层面脾脏;胰腺头部及钩突完全被脂肪密度替代,胰腺体、尾部实质内散在斑片状低密度影(图1 a ),CT 值为-70-90 HU,残余正常胰腺实质增强扫描有明显强化。MRI检查:正、反相位图像上肝脏信号相差较大;胰腺形态、大小正常,脂相上显示胰腺头部及钩突部完全为脂肪信号,胰腺体尾部实质内散在斑片状高信号影,压脂相上可见在脂相上呈高信号的区域信号明显减低(图1bc);增强扫描示残余胰腺实质明显强化,主胰管未见扩张;T2 WI 上胆总管下段可见数个结节状充盈缺损。临床诊断为胆总管下段结石,行 ERCP 取石术及对症治疗后好转。
