摘要
目的采用兔作为动物模型,评估三种不同的抗尿失禁合成吊带原位植入后的宿主反应及安全性。方法 45只清洁级新西兰大白兔被随机分为3组,每组15只,A组采用三维静电仿生聚偏二氟乙烯(polyvinylidene fluoride,PVDF)吊带,B组采用聚丙烯(polypropylene,PP)吊带,C组采用单丝编织聚偏二氟乙烯吊带,分别将三种吊带材料原位植入兔阴道与尿道间隙,于术后4、12和26 w进行观察。结果 3组动物术后均无排尿异常情况,大体观察各组材料均与植入前差异不大,各组材料均未出现侵蚀和暴露现象。A组中形成薄层纤维包裹,轻度炎性细胞浸润,大量血管再生,有少量细胞长入材料内部;B组和C组中材料周围形成较厚的纤维包裹,伴较少炎性细胞浸润,长期观察见增生的纤维组织局部长入周围组织。3组在4 w、12 w和26 w的组织病理学评分比较差异均无统计学意义(P>0.05)。结论三种吊带原位植入后均较安全。三维仿生PVDF吊带可能成为一种治疗女性压力性尿失禁的理想吊带。
Objective To evaluate the host tissue reaction and safety in a rabbit model after in situ implantation of three different synthetic anti - incontinence slings. Methods A total of 45 clean female New Zealand rabbits were randomized into three groups with 15 in each group, group A used three- dimensional electrospinning Polyvinylidene fluoride (PVDF) sling, (group B used polypropylene (PP) sling, and group C used traditional technique PVDF sling. In each group, a piece of implant was placed into the space between urethra and vagina of the rabbits. Animals were sacrificed at week 4, 12, 26 after implantation for gross observation and histopathologic study. Results Postoperative abnormal urination, sling erosion or retraction were not observed in 3 groups. No obvious difference was observed in 3 groups under gross observation before and after implantation. Minimal fibrosis as well as mild inflammatory reaction and rich angiogenesis were observed in group A. In goup B and C, thicker fibers encompassed the implants and penetrated into the surrounding tissue under a long term observation. However, the histopathologic grading showed no significant difference in week 4, week 12 or week 26 among three groups (P 〉 0. 05 ). Conclusion The 3 different slings were all safe in rabbits' body tissue with good biocompatibility. Three - dimensional electrospinning PVDF sling may be a promising synthetic sling for the treatment of female SUI.
