糖尿病合并肺结核患者TNF-α、IFN-γ及IL-10的表达及意义

Expression and significance of TNF-α,IFN-γ and IL-10 in patients of diabetes complicated with tuberculosis

目的探讨糖尿病合并肺结核患者的血清白细胞介素-10(IL-10)、γ干扰素(IFN-γ)及肿瘤坏死因子-α(TNF-α)的变化及意义。方法随机选取我院120例糖尿病合并肺结核患者为A组(根据病灶范围分为A1组60例、A2组40例和A3组20例,根据影像诊断分为AⅠ组50例,AⅡ组70例),160例单纯肺结核患者为B组(其中根据病灶范围分为B1组80例、B2组45例和B3组35例,根据影像诊断分为BⅠ组65例,BⅡ组95例);115例糖尿病患者为C组,120例正常健康对照者为D组。通过检测血清中TNF-α、IFN-γ及IL-10 m RNA表达量,血清TNF-α、IFN-γ及IL-10蛋白含量,并比较分析。结果 A组与B组患者血清TNF-α、IFN-γ及IL-10的m RNA表达和蛋白含量平均值均高于C组和D组,均有显著性统计学意义(P<0.05),并随着病灶范围增加TNF-α、IFN-γ及IL-10的m RNA表达和蛋白含量也同步增高,均有统计学意义(P<0.05),其中A组内部各组分的患者TNF-α、IFN-γ及IL-10的的m RNA表达和蛋白含量增高均明显高于B组内部各组分的患者TNF-α、IFN-γ及IL-10的的m RNA表达和蛋白含量,均有统计学意义。结论肺结核患者外周血TNF-α、IFN-γ及IL-10的m RNA表达和蛋白含量存在着明显差异,其中糖尿病合并肺结核患者变化更加显著,TNF-α、IFN-γ及IL-10在糖尿病合并肺结核的发病和进展中起着一定的作用。

Objective To explore the change of interleukin-10 （IL-10）,interferon γ（IFN-γ） and tumor necrosis factor-alpha （TNF-α） in patients of diabetes complicated with tuberculosis and its significance. Methods 120 diabetic patients with tuberculosis in our hospital were randomly selected for group A （60 patients in group A1,40 patients in group A2 and 20 patients in the group A3 according to area of lesion.And 50 patients in group A Ⅰ ,70 patients in group A Ⅱ according to imaging diagnosis）.160 patients with only tuberculosis were for group B （80 patients in group B1,45 patients in group B2 and 35 patients in group B3 according to area of lesion.And 65 patients in group B Ⅰ ,95 patients in group B Ⅱ according to imaging diagnosis）.115 diabetic patients in group C,and 120 healthy people in group D.The mRNA expression quantities and protein levels of TNF-α,IFN-γ and IL-10 in serum of patients in each group were detected and compared. Results The average mRNA expression quantities and protein levels of TNF-α,IFN-γ and IL-10 in serum of patients in group A and B were both higher than that in group C and D with obvious statistical significant （P 〈 0.05）,and increased with the expansion of the area of lesion,which showed significant difference （P 〈 0.05）.The increase of mRNA expression quantities and protein levels of TNF-α,IFN-γ and IL-10 in serum of patients in group A was significantly higher than that in the group B （P 〈 0.05）. Conclusion The mRNA expression quantities and protein levels of TNF-α,IFN-γ and IL-10 in serum of patients with tuberculosis have obvious difference,which of patients of diabetes complicated with tuberculosis has more significant change.TNF-α,IFN-γand IL-10 play roles in the morbidity and progression of diabetes complicated with tuberculosis.
