Transpalpebral Diaton眼压计与非接触式眼压计测量眼压的差异及影响因素研究 被引量:6

摘要 目的:比较Transpalpebral Diaton 眼压计(TDT)与非接触式眼压计(NCT)在正常人群中眼压测量值的差异,探讨影响两种眼压计测量值的因素。方法选择2014年4—7月南昌大学第一附属医院眼科门诊就诊的眼压在正常范围内的非青光眼患者100例(200只眼),将患者按照就诊顺序分为A组和B组,各50例。 A组采用TDT测量眼压, B组采用NCT测量眼压。同时检查裸眼视力、角膜曲率、屈光度、角膜中央厚度(CCT)、眼轴、散光度及泪膜四项〔泪膜破裂时间( BUT)、角膜荧光素染色( FL)、基础泪液分泌试验( SIT)及泪河高度〕。结果两组裸眼视力、角膜曲率、屈光度、 CCT、眼轴、散光度及泪膜四项检查指标比较,差异均无统计学意义( P>0.05)。 A组眼压、左眼眼压、右眼眼压均低于B组(P<0.05)。两组CCT<500μm者眼压比较,差异无统计学意义(P>0.05); A组CCT 500~550μm者和>550μm者眼压均低于B组(P<0.05)。 A组眼压与年龄呈正相关(P<0.05); B组眼压与年龄呈正相关,与CCT、散光度呈负相关( P<0.05)。直线回归结果显示,年龄是影响TDT眼压计测量值的因素( P<0.05);年龄、 CCT、散光度是影响NCT眼压计测量值的因素( P<0.05)。结论 TDT测量值比NCT测量值偏低,且受年龄影响,与CCT和散光度无关。 Objective To compare the difference between Transpalpebral Diaton tonometer (TDT) and Non - contact tonometer (NCT) in the measured value of intraocular pressure ( IOP), and to explore the influencing factors for the measured value by the two kinds of tonometers. Methods We enrolled 100 patients who had normal intraocular pressure, had no glaucoma and received treatment in the Department of Ophthalmology of the First Affiliated Hospital of Nanchang University from April to July 2014 as the subjects. According to their order of visiting, we divided the subjects into two groups: group A (n = 50) and group B (n = 50). In group A, the IOP measurement was conducted using TDT, and in group B, the IOP measurement was conducted using NCT. We checked a series of indicators, including UCVA, corneal curvature, refractive diopter, CCT, ocular axis length, astigmatism degree, four items of tear film (BUT, FL, SIT and TMH). Results There were no significant differences (P 〉 0. 05 ) between the two groups in UCVA, corneal curvature, refractive diopter, CCT, ocular axis length, astigmatism degree and four items of tear film. Group A was lower (P 〈 0. 05 ) than group B in IOP, left eye I0P and right eye IOP. Two groups were not significantly different (P 〉 0.05) in IOP of the subjects with CCT 〈 500 μm; group A was lower ( P 〈 0. 05 ) than group B in IOP of the subjects with CCT within 500 - 550 or CCT 〉 550 μm. The IOP of group A was positively correlated with age (P 〈 0.05 ); the IOP of group B was positively correlated with age and was negatively correlated with CCT and astigmatism degree (P 〈 0. 05 ). The linear regression analysis showed that age was an influencing factor for the measured value of IOP by TDT ( P 〈 0. 05 ), and age, CCT and astigmatism degree were influencing factors for the measured value of IOP by NCT ( P 〈 0. 05 ). Conclusion The measured value of IOP by TDT is lower than NCT, and it is influenced by age and unrelated with CCT and astigmatism degree.
