综合疗法治疗银屑病43例临床研究

Clinical Observation of Combined Modality Therapy on 43 Patients with Psoriasis Vulgaris

目的:探讨中医整体观指导下的综合疗法治疗寻常型银屑病的临床疗效。方法:将80例银屑病患者随机分为治疗组43例和对照组37例。两组患者均口服自拟凉血消银汤,外洗清热外洗散,外擦紫龙膏,治疗组在药物的基础上同时进行健康处方、安神养形法、音乐疗法。治疗4周后评价疗效。结果:治疗组总有效率为79.1%,对照组总有效率为75.7%,治疗组优于对照组,但差异无统计学意义(P>0.05);两组患者治疗后PASI积分均较治疗前明显下降,差异均有统计学意义(P<0.01);两组患者治疗后PASI积分比较,差异无统计学意义(P>0.05);两组心烦、瘙痒症状评分均改善(P<0.05);SCL-90评分:治疗组治疗后人际关系、敌对、强迫、抑郁、焦虑等5个因子评分均明显下降,差异有统计学意义(P<0.05);对照组9个因子评分治疗前后比较,差异均无统计学意义(P>0.05)。结论:综合疗法治疗寻常型银屑病临床疗效确切,可改善患者的心理健康状况,提高患者的生活质量。

Objective： To study the clinical efficacy of combined modality therapy on psoriasis vulgaris, based on the guidance of Chinese medicine-the holistic concept. Methods： The 80 patients with psoriasis vulgaris were randomely divided into treatment group and control group, 43 cases and 37 cases respectively in each. Patients in both of the two groups took Liangxuexiaoyintang （凉血消银汤） and externally washed the sick spot with Chinese medicine Qingrewaixisan （清热外洗散） and Zilonggao （紫龙膏）. Patients in treatment group were given health prescription, the method of soothing mind and music therapy. The results of the two groups were assessed in 4 weeks. Results： The effective rate was 79.1% in treatment group and 75.7% in control group. The treatment group showed better efficacy than the control group, but there was no statistically significant differ- ence（P〉0.05）; the PASI scores remarkably decreased in both groups after the therapy, and there was significant difference （P〈0.01）; there was no difference between the clinical data on the PASI score after the therapy（P〉0.05）. The symptoms of unease and pruritus were improved in both two groups（P〈0.05）. The results for the SCL-90 score is shown as follows： the 5 kinds scores of interpersonal relationship, hostility, compelling, depression, anxiety were reduced, with significant difference （P〈0.05） . The scores for the 9 factors in control group showed no statistically significant difference before and after treatment （P〉0.05）. Conclusion： The combined modality therapy can provide accurate clinical efficacy for psoriasis vulgaris, effectively improve the mental health as weel as the life quality of patients.
