疤痕子宫再次妊娠出现前置胎盘的临床结局分析

Analysis of Clinical Outcome of Placenta Previa During Re- pregnancy with Scarred Uterus

目的分析疤痕子宫再次妊娠出现前置胎盘的临床结局。方法选择2012年5月至2014年5月期间我院收治的45例疤痕子宫再次妊娠合并前置胎盘的孕产妇作为研究组,选择同期收治的45例疤痕子宫再次妊娠孕产妇作为对照组。观察对比两组产后出血量、分娩时间、手术费用、子宫切除率、产后出血率及新生儿窒息率。结果研究组2 h内的出血量及24 h内的出血量显著高于对照组(P<0.05);分娩时间及手术费用显著高于对照组(P<0.05);子宫切除率、产后出血率、新生儿窒息率显著高于对照组(P<0.05)。结论疤痕子宫再次妊娠合并前置胎盘可以提高围生期母婴并发症概率,临床应给予足够的重视,着重强调孕期产检的重要性,不断提高手术技巧,以此保障母婴健康。

Objective To analyze the clinical outcome of placenta previa during re - pregnancy with scarred uterus. Method 45 pregnant women with placenta previa during re -pregnancy with scarred uterus who were admitted into our hospital from May 2012 to May 2014 were selected as observation group, 45 pregnant women during re -pregnancy with scarred uterus admitted during the same period as control group. The amount of postpartum bleeding, delivery time, surgery costs, hysterectomy rate, incidence of postpartum hemorrhage, and neonatal asphyxia rate of the two groups were observed and compared. Results The amount of postpartum bleeding of the observation group within 2 hours and 24 hours were evidently higher than those of the control group （ P 〈 0.05 ） , the delivery time and surgery cost of the observation group were evidently higher than those of the control group （ P 〈 0.05 ） , hysterectomy rate, in- cidence of postpartum hemorrhage, and neonatal asphyxia rate of the observation group were evidently higher than those of the control group （ P 〈 0. 05 ）. Conclusion The placenta previa during re - pregnancy with scarred uterus may increase the complication possibil- ity of mothers and babies in perinatal period, which should be given enough clinical attention. We should emphasize the importance of antenatal examination and continuously improve surgical techniques so as to safeguard both mothers and babies.
