睾丸素/雌二醇对低剂量克罗米芬治疗特发性少精弱精症效果的预测价值

The predictive value of testosterone/estradiol in the effect of low doses of clomiphene in the treatment of idiopathic asthenospermia

目的:探讨睾丸素/雌二醇(T/E2)值对低剂量克罗米芬治疗特发性少精弱精症效果的预测价值。方法:选择特发性少精弱精症患者60例,予小剂量克罗米芬治疗,观察治疗效果,比较治疗前后的精子密度、精子活动率、促卵泡激素(FSH)、促黄体生成素(LH)、T、E2及T/E2值的变化。结果:克罗米芬治疗特发性少精弱精症的临床治愈率为26.67%,总有效率为76.67%;治疗后患者精子密度、精子活动率、FSH、LH、T和T/E2值均较治疗前明显升高,而E2显著下降(P<0.01)。结论:精子密度、精子活动率、FSH、LH值是低剂量克罗米芬治疗特发性少精弱精症疗效评价的良好指标,治疗过程中T/E2值变化也很明显,可以与上述指标一起作为克罗米芬有效性综合评估的指标。

Objective：To explore the predictive value of testosterone/estradiol（T/E2 ） in the effect of low doses of clomiphene in the treatment of idiopathic asthenospermia. Methods： Sixty patients with idiopathic asthenospermia were treated with small dose of clomiphene, and the treatment effect was observed. The sperm density, sperm activity rate, and the levels of follicle-stimulating hormone （FSH）, luteinizing hormone （LH）, T, E2 and T/E2 between before and after treatment were compared. Results：The clinical cure rate and total efficiency of the clomiphene in the treatment of idiopathic asthenospermia were 26.67% and 76.67%, respectively. Compared with before treatment, the sperm density, sperm activity rate, and the levels of FSH, LH,T and T/E2 increased significantly, but the E2 level decreased significantly after treatment （P 〈 0.01 ）. Conclusions：The sperm density, sperm activity rate, and the levels of FSH and LH are good indicators in evaluating the curative effect of low dose clomiphene in the treatment of idiopathic asthenospermia. The change of T/E2 together with other indicators can also be a comprehensive evaluation index in evaluating the effect of clomiphene.
