扶正祛毒汤治疗结直肠癌术后永久性结肠造口临床研究

The Curative Effect and Quality of Life Observation of Chinese Medicine Intervention in Permanent Colon Colostomy After Colorectal Cancer Operation

目的:观察扶正祛毒汤联合FOLFIRI化疗方案治疗结直肠癌术后永久性结肠造口的临床疗效。方法:选取本院2009年3月—2012年7月肛肠外科就诊的结直肠癌患者180例,随机分为治疗组和对照组。治疗组88例采用扶正祛毒汤联合FOLFIRI方案进行化疗,对照组92例只用FOLFIRI方案化疗。随访记录患者临床疗效、不良反应、复发与转移以及生存质量评分。结果:治疗组有效率为88.64%,显著高于对照组的36.96%(χ2=51.1,P<0.001);治疗组骨髓抑制、肝功能损害、胃肠道反应、脱发和体质量下降不良反应发生率显著低于对照组(P<0.05)。ORTC QLQ-C30量表中,治疗组在功能部分(除躯体和认知功能),症状部分和单项量表方面(除经济困难)均优于对照组(P<0.05)。但2 a复发和转移率,两组无显著性差异(P>0.05)。结论:扶正祛毒汤辅助化疗能显著提高结直肠癌术后永久性结肠造口患者的有效率,改善化疗引起的不适症状,提高患者术后生存质量。

Objective： To observe the clinical curative effect of self-made Traditional Chinese medicine in combination with FOLFI- RI chemotherapy on permanent colon colostomy after colorectal cancer operation. Methods： One hundred and eighty patients with colorectal cancer who were treated in the colorectal surgery department of our hospital between Mar. 2009 and Jul. 2012 were randomly divided into the treatment group and the control group, with 88 and 92 cases in each group respectively. The ones in the treatment group were given self-made Traditional Chinese medicine in combination with FOLFIRI chemotherapy, while the ones in the control group were only given FOLFIRI chemotherapy. Through the follow-up, the clinical curative effects, adverse reactions, reoccurrence, transfer, survival quality of both groups were recorded. Results： The total effective rate of the observation group was 88.64% , which was much higher than that （36.96%） of the control group（ 2 = 51.1 ,P 〈 0.001 ）. The occurrence rate of bone marrow depression, liver dysfunction, gastrointestinal disorders, hair loss, and weight decreased, anti adverse reactions of the observation group was much lower than that of the control group （ P 〈 0.05 ）. According to ORTC QLQ-C30 scale, in terms of functions （ except body and cognitive function） , symptoms and single scale （except economic difficuhy） , the observation group was much better than those of the control group（ P 〈0. 05 ）. But in terms of 2-year reoccurrence and transfer rates, there were no significant difference （ P 〉 0.05 ）. Conclusion： Traditional Chinese medicine in combination with FOLFIRI chemotherapy can im- prove the clinical curative effect of patients with permanent colon cotostomy after colorectal cancer operation, better uncomfortable symptoms due to chemotherapy, and the life quality.
