中西医结合治疗糖尿病皮肤瘙痒临床研究

The Clinical Research into Diabetes Skin Itch Treated with Therapy of Integrated Traditional Chinese and Western Medicine

目的:观察乌蛇驱风汤加减联合氯雷他定片治疗糖尿病皮肤瘙痒的临床疗效以及对血清β2微球蛋白(β2-MG)和甲状旁腺素(parathyroid hormone,PTH)的影响。方法:将糖尿病皮肤瘙痒患者102例按照数字表法随机分为对照组和治疗组各51例,对照组采取氯雷他定片内服治疗,每次10 mg,日1次,治疗组在对照组治疗的基础上采取乌蛇驱风汤加减治疗,日1剂,早晚两次服用。两组患者均连续治疗4周。比较两组患者治疗后2周和4周瘙痒评分,记录两组不良反应发生率,分析两组患者临床疗效,检测两组血清β2-MG和PTH水平。结果:治疗组治疗后2周和4周患者瘙痒评分均分别低于对照组(P<0.01);治疗组有效率为94.12%,优于对照组的76.47%(χ2=5.002,P<0.05);治疗组不良反应发生率为3.92%,对照组不良反应发生率为19.61%,治疗组明显少于对照组(χ2=4.628,P<0.05);治疗组治疗后患者血清β2-MG和PTH水平均明显低于对照组,比较差异有统计学意义(P<0.01)。结论:乌蛇驱风汤加减联合氯雷他定片治疗糖尿病皮肤瘙痒疗效显著。

Objective： To observe the clinical curative effect of Modified Wushe Qufeng Decoction （MWSQFD） in combination with loratadine tablets on diabetes skin itch, and the influence on serumβ2-MG and parathyroid hormone, （PTH）. Methods： One hundred and two patients with diabetes skin itch were divided into the control group and the treatment group according to random number table method, with 51 cases in each group. The ones in the control group were given loratadine tablets for oral adminis- tration, l0 mg each time, and once daily, while the ones in the treatment group were added MWSQFD for oral administration, one dose a day, and taking decoction for 2 times in the morning and in the evening. Both groups were treated for 4 weeks continu- ously. Then ＇after 2 weeks＇and 4 weeks＇treatment respectively, the respective itch score of both groups was compared; Their occurrence rates of adverse reactions were recorded; Their clinical curative effects were analyzed; And their serum β2-MG level and PTH level were detected. Results： After 2 weeks＇and 4 weeks＇treatment respectively, the itch score of the treatment group was lower respectively than that of the control group （P 〈0.01 ）. The effective rate of the treatment group was 94.12%, which was better than that （76.47%） of the control group （ χ2 = 5. 002, P 〈 0.05）. The occurrence rate of the treatment group was 3.92%, which was lower than that（ 19.61% ） of the control group（ χ2 = 4. 628 ,P 〈 0.05 ）. After the treatment, serum β2-MG level and PTH level of the treatment group were much lower than those of the control group; And the differences between them had statistical significance （ P 〈 0.01 ）. Conclusion： MWSQFD in combination with loratadine tablets has remarkable clinical curative effect on diabetes skin itch.
