儿童Poland综合征CT表现及CT影像学分级研究

CT Findings and CT-Based Rating Scale of Poland Syndrome in Children

目的评价CT重建技术在儿童Poland综合征影像学诊断和分级中的价值。方法回顾性分析10例儿童Poland综合征患儿的临床资料,比较其影像学检查结果。结果 10例患儿中年龄最大者11岁10个月,最小者8个月。患儿均因胸廓外观异常就诊,无其他心脏、呼吸、肾脏和脊椎症状。CT平扫均可看到一侧胸大肌和胸小肌完全或部分缺失,4例患儿CT平扫及重建结果显示合并出现同侧胸壁Ⅱ~Ⅳ肋骨发育异常。结论 CT平扫结合三维重建能为检出和诊断儿童Poland综合征提供全面准确的影像信息,本研究通过影像表现提出的分级方法能够为儿童Poland综合征临床治疗方案及适宜治疗时期的选择提供必要依据。

Objective Aim To evaluate the value of CT image reconstruction in the diagnosis and rating scale of pediatric Poland syndrome. Methods Clinical data and image results of the pediatric Poland syndrome cases in our institution between June 2009 and June 2014 were reviewed. Results Among the patients 8 were boys,2 were girls,aged between8 months old to eleven years and 10 months old. The majority of patients were manifested with abnormal thoracic appearance,with no other complain of cardiac,respiration,kidney or vertebral. CT scan results showed complete or partial absence of the pectoralis muscles,and reconstruction of CT images displayed that there were partial agenesis of the ribs in 4patients. 10 patients were outpatients with no further treatment. Conclusion CT scan and images reconstruction possess important value in the diagnosis of pediatric Poland syndrome and benefit for the radiological ranking.
