抑郁障碍共病高血压患者全脑功能损害的静息态功能磁共振成像低频振幅研究 被引量:7

摘要 目的:应用静息态功能磁共振成像技术,探讨抑郁障碍共病高血压患者在静息态下的脑功能损害特点。方法:共病组为30例抑郁障碍共病高血压患者,对照组为性别、年龄及受教育程度匹配的30例单纯抑郁障碍患者和30例健康被试。对所有被试进行静息状态下全脑功能磁共振扫描,采用低频振幅作为测量指标,比较三组被试的静息态脑功能差异。通过方差分析得出三组低频振幅差异有统计学意义的脑区,计算三组差异脑区的低频振幅均值并进行组间比较。结果:三组被试低频振幅差异有统计学意义的脑区包括左小脑、左舌回、右内侧额上回、右眶部额上回(P<0.05,已校正);组间比较显示共病组在左小脑和右内侧额上回的低频振幅值与抑郁障碍组差异有统计学意义(P<0.01),共病组和抑郁障碍组在左舌回和右眶部额上回的低频振幅值与健康对照组差异有统计学意义(P<0.01)。结论:本研究提示抑郁障碍共病高血压与单纯抑郁障碍患者均存在局部脑区功能异常,共病患者某些脑区异常更明显。 Objective: To examine the brain function of patients with depressive disorder co-morbid with hypertension by using resting state functional magnetic resonance imaging (fMRI) technology. Methods: Thirty patients with hypertension and co-morbid major depressive disorder (comorbidity group), 30 patients with major depressive disorder (depression group), and 30 health controls (HC group) were recruited. The subjects were matched for sex, age and years of education in three groups. All participants underwent the resting state fMRI. Amplitude of low frequency fluctuation (ALFF) approach was used to explore the differences of resting-state brain functiorL The differences of ALFF of brain areas were tested by ANOVA, then post-hoc analysis was used to test differences between groups. Result: The ALFF values of left cerebellum, left lingual gyrus, right superior medial frontal gyrus,right orbital frontal gyrus were significantly different among the three groups (P 〈 0.05, corrected). The ALFF values of left cerebellum and right superior medial frontal gyrus were significantly different between comorbidity group and depression group (P 〈0. 01). The ALFF values of left lingual gyrus and right orbital frontal gyrus of comorbid- ity and depression groups were significant different from HC group (P 〈 0. 01). Conclusion: The results indicate that the function of some areas of brain are impaired in patients with depression, and the impairments may be seve- rer in patients with co-morbid hypertension.
