肛瘘的磁共振成像诊断与术中对比研究

Comparative study on anal fistula diagnosed by MRI and intraeperative results

目的探讨磁共振成像(MRI)在肛瘘诊断中的应用价值及表现,以提高对本病的术前诊断水平。方法回顾性分析35例肛瘘的MRI表现,并与术中结果对比。结果在35例肛瘘中,应用Parks分类法,分4型,肛管括约肌间型22例,经肛管括约肌型7例,肛管括约肌上型4例,肛管括约肌外型2例;MRI可直接显示瘘管内口、外口及位置,瘘管的走行及其分支,与术中结果对比,诊断符合率为91%。结论 MRI能准确评估肛瘘形态、走行及与肛管内外括约肌、肛提肌的关系,是一种无创伤性、高准确性的检查方法,对临床选择正确的治疗方法具有重要意义。

Objective To study the performance and value of MRI in the diagnosis of anal fistula,and to im- prove the preoperative diagnosis level. Methods MRI performance of 35 cases with anal fistula were retrospectively analyzed, and were compared to postoperative. Results By Parks classification, 35 cases with anal fistula were divided into four types, there were 22 cases of inter-sphincteric, 7 cases of trans-sphincteric, 4 cases of supra-sphincteric and 2 cases of extra-sphincteric. MRI can directly show fistula in internal opening, outside opening and position, direction and branch of fistula, compared with the results of intraoperative, diagnostic coincidence rate was 91%. Conclusion MRI can accurately assess form, course of anal fistula, or relationship with both internal and external anal sphincter and perianal, which is a kind of no traumatic, inspection method with high accuracy, has great significance to the clinical treatment.
