血清胰岛素及胰岛素抵抗与子宫内膜癌发生的相关性研究探讨 被引量:5

摘要 目的对血清胰岛素及胰岛素抵抗与子宫内膜癌发生的相关性进行研究。方法以2012年3月至2014年3月在我院住院治疗的97例子宫内膜癌患者作为观察组,以98名健康体检的女性作为对照组,所有患者均符合伦理学要求,征得患者同意,检测两组入选对象空腹血糖及胰岛素水平,并计算胰岛素抵抗指数,探讨其与子宫内膜癌发生的相关性。结果无论糖尿病患者或非糖尿病患者,观察组空腹胰岛素、HOMA-IR水平明显高于对照组,相比差异有统计学意义(P<0.05),而两组空腹血糖相比差异无统计学意义(P>0.05);FIN、HOMA-IR与组织学分级、FIGO分期、浸润肌层深度、雌激素受体及孕激素受体无明显相关性,相比差异无统计学意义(P>0.05)。结果子宫内膜癌的发生、发展是一个复杂的过程,血清胰岛素的水平不能直接反映子宫内膜癌的发展情况,但可能是导致子宫内膜癌发生的独立危险因素,可能是子宫内膜癌发生的机制之一,有待于进一步研究。 Objective To study the correlation of serum insulin and insulin resistance with endometrial cancer.Methods 97 cases of patients with endometrial cancer treatment as the observation group and 98 healthy women as control group were selected for this study. The fasting plasma glucose and pancreatic hormone levels in all subjects were detected and the insulin resistance index were calculated. Results The fasting insulin and HOMAIR levels in patients were significantly higher than that in the control group(P〈0. 05),but there was no significant difference on fasting blood glucose between two groups(P〈0. 05). There was no significant correlation between the FIN and HOMA-IR levels with histological grade,FIGO stage,myometrial invasion depth,estrogen receptor and progesterone receptors(P〈0. 05). Conclusion The occurrence and development of endometrial cancer is a complex process. The serum level of insulin cannot directly reflect the development of endometrial cancer,but may be the cause of independent risk factors for endometrial cancer. It may be one of the mechanisms of endometrial cancer and need to be further studied.
