三维适形放疗同步化疗治疗恶性脑胶质瘤

Three-dimensional Conformal Radiotherapy Combined with Concurrent Chemotherapy in the Treatment of Malignant Brain Glioma

目的比较恶性脑胶质瘤术后三维适形放疗(3D-CRT)与3D-CRT同步替莫唑胺(TMZ)的疗效及毒副反应。方法 113例恶性脑胶质瘤术后患者分为2组,研究组68例接受3D-CRT同步TMZ治疗,对照组45例仅接受单纯3D-CRT治疗。结果研究组和对照组有效率分别为69.1%和33.3%,1、2、3 a生存率分别为67.6%、47.1%、33.8%和51.1%、24.4%、15.6%(P均<0.05)。研究组Ⅰ、Ⅱ度胃肠道反应、白细胞减少的发生率高于对照组(P均<0.05)。结论 3D-CRT同步TMZ可显著提高恶性脑胶质瘤术后肿瘤控制效果,且患者耐受性良好。

Objective To compare the efficacy and toxicities of three-dimensional conformal radiotherapy （3D- CRT） and 3D-CRT combined with concurrent temozolomide （TMZ） in the treatment of postoperative malignant brain glioma. Methods The 113 patients with malignant brain glioma were divided into two groups,68 patients of the study group received 3 D-CRT combined with concurrent TMZ, and 45 patients of the control group received 3 D- CRT alone. Results The effective rates of the study group and the control group were 69.1% and 33.3% ,respec- tively （P 〈 0.05 ） ; the 1,2,3-year survival rates were 67.6% ,47.1% ,53.8% and 51.1% ,24.4%, 15.6%, respec- tively （ P 〈 0.05 ）. The incidences of grade Ⅰ and Ⅱgastrointestinal reaction and leukopenia in the study group were higher than those in the control group （P 〈 0.05 ）. Conclusion 3D-CRT combined with concurrent TMZ can signifi- cantly improve the postoperative tumor control effect of malignant brain glioma,and the toxicities can be tolerated.
