妊娠期高血压病患者脐血流S/D值与围生儿结局的临床分析

Clinical analysis on the umbilical artery S/D of hypertensive disorders in pregnancy with perinatal infant outcomes

目的研究妊娠期高血压疾病患者脐血流S/D值与围生儿结局的相关性。方法 (1)随机选择妊娠期高血压病患者110例及无此合并症的正常孕妇60例,采用彩色多普勒超声诊断仪检测正常妊娠及妊娠期高血压疾病患者脐动脉血流S/D值,比较两者脐血流S/D值差异;(2)妊娠期高血压病患者按S/D<2.5、2.5≤S/D<3.0、S/D≥3.0分为三组,分析不同S/D值与围生儿结局(低体重儿、早产、新生儿窒息、围生儿死亡)之间的关系。结果妊娠期高血压病组脐血流S/D值高于正常妊娠组(P<0.05);胎儿脐血流S/D值与低体重儿、早产、新生儿窒息呈正相关,差异有统计学意义(P<0.05)。结论妊娠期高血压病可导致脐动脉血流阻力增加,影响围生儿的预后,动态监测脐血流S/D值,发现异常及时处理。

Objective To study relevance of patients with hypertensive disorders complicating pregnancy umbilical artery S/D and perinatal infant outcomes. Methods （1） A retrospective study design was used with 110 cases patients of hypertensive disorders in pregnancy and 60 cases patients of normal pregnancy who received color doppler ultra- sound to measure the umbilical artery S/D value, and different umbilical artery S/D value were compared. （2） All the Hypertensive disorders in pregnancy were classified into three groups,i.e. S/D〈2.5,2.5 ≤S/D〈3.0 and S/D ≥ 3.0. The relationship between different S/D values and perinatal infant outcomes（low birth weight infant,premature birth,neonatal asphyxia,perinatal mortality） were analyzed. Results The umbilical artery S/D value in hypertensive disorders compli- catinng pregnancy was higher than normal pregnancy （P〈0.05）. Low birth weight infants,premature birth,neonatal as- phyxia and perinatal mortality were positively correlated with different S/D value ,and it were statistically significant （P〈0.05）. Conclusion Hypertensive disorders in pregnancy lead to increased umbilical artery resistance, which im- pact perinatal infant prognosis. Monitoring umbilical artery blood value, abnormal timely manner.
