右美托咪定与丙泊酚在直肠癌磁共振检查中的效果对比

Comparison of two anesthetic methods in rectal cancer MRI examination

目的探讨老年直肠癌患者磁共振（MRI）检查的麻醉管理。方法2010年7月-2013年7月48例收治的直肠癌患者。其中，男32例，女16例，年龄56~74岁，平均67．74岁，在麻醉下行直肠MRI检查。按照麻醉用药的不同将其分为右美托咪定组（D组）、丙泊酚组（P组）。镇静和麻醉过程连续监测患者的生命体征。对各组用药的成功率、维持时间、苏醒时间、检查过程中心率、呼吸频率及患者满意度进行分析。结果两组用药后行检查的成功率均为100％。P组较D组维持时间略短，呼吸频率减慢；D组较P组心率明显减缓，但均在正常范围之内。结论丙泊酚或右美托咪定麻醉均能满足患者直肠MRI检查的要求，但丙泊酚麻醉易发生呼吸抑制，右关托咪定麻醉是更安全的方法。

[ Objective ] To investigate the anesthesia management of magnetic resonance image （MRI） inspection for the gerontal patients with rectal cancer. [ Methods ] Forty-eight patients with rectal cancer took rectal MRI in- spection with anesthesia from July 2011 to January 2013, who aged from 56~74 years old with an average age of 67.74 years old, 32 cases were male and 16 cases were female. According to the different anesthesia drugs all the patients were divided into dexmedetomidine group （group D） and propofol group （group P）. During sedation and anes- thesia vital signs of patients in the two groups were monitored continuously. Finally the successful treatment rate, holding time, recovery time, heart rate and respiratory rate during the checking process, and patients satisfaction of each group were analyzed. [Results] The successful rate of the two groups were 100% respectively. Maintaining time was slightly shorter and respiratory rate was slower in P group than that in D group; however heart rate was sig- nificantly slower in D group than that in P group, but both were in the normal range. [ Conclusions ] Either propofol or dexmedetomidine, the two anesthesia methods can meet the need of rectal MRI inspection, but the propofol anes- thesia can cause respiratory depression easily, thus dexmedetomidine is a better choice.
