慢性中耳炎真菌感染的临床分析

Diagnosis and treatment of chronic otitis media with fungal infections

目的探讨慢性中耳炎伴真菌感染的原因和具体诊断方法以及各种治疗方法的疗效。方法选择2010~2013年我院收治的50例慢性中耳炎伴真菌感染的患者的资料,按照其接受的治疗方法分为局部治疗组、局部＋口服药物治疗组、手术＋口服药物治疗组3组,分析评价各组的治疗效果。结果局部用药24例,治愈13例,有效6例,治愈率为54.17%,总有效率为79.17%;局部＋口服用药18例,治愈12例,有效5例,治愈率为66.67%,总有效率为94.44%;手术治疗8例,全部治愈,治愈率为100.00%。局部用药与手术治疗相比较、局部用药与局部＋口服用药相比较,其治愈率和总有效率差异具有统计学意义（P〈0.05）。结论局部联合口服药物治疗能够显著提高患者治疗效果,因此对口服药没有过敏或禁忌的患者应当推荐使用联合用药,对于感染严重以及外耳道开口狭小的患者应当主张其积极进行手术治疗。

Objective To investigate the effect of fungal infections associated with chronic otitis media causes and spe- cific diagnostic modalities as well as a variety of treatment methods. Methods Collected from 2010 to 2013 in our hos- pital 50 cases of patients with chronic otitis media accompanied by basic information fungal infections, according to the treatment of their acceptance into local treatment groups, local ＋ oral medication group, surgery plus oral medica- tion group 3 group,the therapeutic effect of each group were evaluated. Results Among 24 cases,cured 13 cases,effec- tive in 6 cases,the cure rate was 54.17%,the total effective rate was 79.17%; 18 cases of topical and oral medication, cured 12 cases,effective in 5 cases,the cure rate was 66.67%, the total efficiency 94.44%; Surgery 8 cases,all cured, the cure rate was 100.00%. Topical and surgical treatment compared to topical and topical and oral medication com- pared to its cure rate and total effective rate were significantly different（P〈0.05）. Conclusion Local combined with oral drug therapy can significantly improve the treatment effect, so no oral drugallergies or contraindicated in patients should be recommended for severe infection and drug combination, external auditory canal opening small patients should advocate the positive operation treatment.
