摘要
目的探讨糖化血红蛋白和尿微量白蛋白联合检测在早期糖尿病肾病中的临床诊断效果及其临床价值。方法对我院收治的109例糖尿病肾病患者资料进行分析,将其设为实验组,选择我院同期健康体检的48例健康者为对照组。根据患者糖化血红蛋白水平将患者分为低值组、中值组和高值组,采用高效液相色谱法检测糖化血红蛋白,免疫比浊法检测尿微量白蛋白,比较两组糖化血红蛋白和尿微量白蛋白指标。结果实验组糖化血红蛋白水平、尿微量白蛋白水平显著高于对照组(P<0.05);低值组糖化血红蛋白水平、尿微量白蛋白水平显著低于高值组(P<0.05);低值组尿微量白蛋白水平与中值组相比差异不显著(P>0.05);实验组患者的尿微量白蛋白水平随着糖化血红蛋白的升高也出现增高趋势,并且两者表现出明显的正相关关系(r=0.491,P<0.05)。结论早期糖尿病肾病患者治疗过程中加强糖化血红蛋白和尿微量白蛋白联合检测效果理想,能够对早期糖尿病肾病患者早期诊断、早期治疗,值得推广应用。
Objective To investigate the clinical value of glycated hemoglobin and combined detection of urinary albu- min in diagnosis of the early diabetic nephropathy. Methods A total of 109 cases of diabetic nephropathy patients ad- mitted to our hospital were seleced as the experimental group, chose 48 healthy subjects in the same period in our hos- pital as the control group. According to glycated hemoglobin levels, 109 patients were divided into low group, median group and high value group, used liquid chromatography college glycated hemoglobin, immunoturbidimetric assay of urinary albumin, glycated hemoglobin and urine microalbuminuria protein targets were compared. Results The glyeated hemoglobin levels, urinary albumin levels of experimental group were significantly higher, the difference was significant (P〈0.05); glycated low levels of urinary albumin levels were significantly lower than the high value group, the difference was significant (P〈0.05); and the level of urinary albumin in the low value group compared with the median value group was not significantly different (P〉0.05); the experimental group with elevated glycated hemoglobin, urinary albu- min levels appear higher trend, and both performance a significant positive correlation (r=0.491, P〈0.05). Conclusion Early treatment of diabetic nephropathy in patients with hemoglobin and strengthen the combined detection of urinary albumin results are satisfactory, it is possible for early early diagnosis and early treatment of diabetic kidney disease, should be introduced.
