临床医生输血不良反应认知情况调查

Evaluating clinicians' knowledge on blood transfusion reactions

目的调查临床医生对于输血不良反应相关知识的掌握程度。方法自行设计临床医生输血不良反应认知调查问卷,经专家审查和修改后在1个月内先后2次对50名临床总住院医师进行预调查,然后采用方便随机、匿名的方式对140名临床医生进行正式调查。结果 2次预调查的复测信度为0.88,第1次预调查的平均区分度0.344,平均通过率0.499。正式调查共发放调查问卷140份,回收115份,回收率82.1%,有效问卷101份,有效率87.8%。得分最低4分,最高20分,平均得分12.58±3.50,正确率57.20%。平均得分在性别、年龄、工作职称、工作时间、每月输血次数、参加培训次数方面差异均有统计学意义(P均<0.05)。得分较低的答题涉及的输血不良反应是输血相关性移植物抗宿主病、输血相关性急性肺损伤、含铁血黄素沉着和全身过敏反应。结论临床医生对于输血不良反应相关知识的掌握程度存在明显的不足,医院管理者要进一步加强对于医护人员输血不良反应相关知识的培训和宣传,改进目前的输血不良反应报告制度。

Objective To investigate the level of knowledge on common blood transfusion reactions among clinicians.Methods A knowledge questionnaire was developed by hematologists and 50 chief residents who had undergone two preliminary investigations within one month. Then,a random sample of 140 anonymous clinicians from a large general hospital were enrolled for the formal investigation. Results The retest reliability of preliminary investigation was 0. 88. The average degree of differentiation was 0. 344. The average passing rate was 0. 499. The response rate was 82. 1%. The efficiency rate of the formal investigation was 87. 8%. The overall knowledge scores of clinicians ranged from 4 to 20 with a possible score of22,and the average total score was 12. 58 ± 3. 50. The correct rate was 57. 20%. There were statistically significant relationship between clinicians’ knowledge and gender,age,years of working,number of blood transfusions per month,number of training on blood transfusion（ P 〈 0. 05,respectively）. Conclusion The level of knowledge of blood transfusion reactions among clinicians is notably deficient.
