摘要
目的:探讨颅内静脉窦血栓形成(CVST)的临床特点及其磁共振静脉成像与64层螺旋CT诊断特点。方法:回顾性分析本院2011年1月-2014年1月收治的CVST患者30例的临床资料。结果:30例颅内静脉窦血栓形成患者中,女性多于男性,急性高于亚急性及慢性,意识障碍、认知功能异常症状发生率明显高于大脑皮层刺激征、运动功能异常和眼部症状的发生率,比较差异均有统计学意义(P<0.05);头颅磁共振静脉成像(MRV)检查示静脉血管、静脉窦变窄或阻塞,血栓形成处血流信号降低甚至消失,呈三角征高或低信号,其中静脉源性脑梗死呈T1WI低信号,T2WI高信号;而静脉源性出血呈T1WI高信号、T2WI高信号。64层螺旋CT显示:静脉窦区示条带样、类圆形、三角形高或低密度影,其中静脉源性脑梗死呈低密度影,而静脉源性脑出血呈高密度影。结论:颅内静脉窦血栓形成好发于女性,以意识、认知功能异常及急性发病为主要临床表现,MRV或CT检查结果为其诊断提供依据。
Objective: To investigate the clinical features of intracranial venous sinus thrombosis and diagnostic analysis of magnetic resonance venography and 64 slice spiral CT on it.Method: The clinical date of 30 patients with CVST in our hospital from Junuary 2011 to January 2014 were retrospectively analyzed.Result: T the 30 cases of intracranial venous sinus thrombosis Datients, female was more than male, acute was more than subacut and chronic, the occurrence rates of conscious disturbance hnd abnormal cognitive function were more than the incidence rates of cerebral cortex stimulation syndrome, abnormal motor function and ocular symptoms, the differences were statistically significant ( P〈0.05 ) .Cranial magnetic resonance venography ( MRV ) examination showed that vein, venous sinus was narrowed down or obstruction.The blood signals were lower or even disappeared, it showed a triangular syndrome with high or low signal.The vein source cerebral infarction showed low signal on T1WI and high signal on T2WI.64 slice spiral CT showed that sinus area was strip sample, quasi-circular or triangle with high or low signal, the vein source cerebral infarction showed low density image and vein source cerebral hemorrhage showed high density image.Conclusion: Intracranial venous sinus thrombosis is good hair in female.Its main clinical manifestations are acute onset and abnormal symptoms in consciousness and cognitive function, MRV or CT results provide the basis for its diagnosis.
