强化降脂对急性期缺血性卒中预后观察

Effects of intensive lipid-lowering on prognosis of acute ischemic stroke

目的:探究强化降脂治疗方案对急性期缺血性卒中预后的影响,为临床研究提供参考依据。方法:选择本院收治的82例急性期缺血性卒中,随机分为实验组和对照组,分别为40例和42例。实验组采用强化治疗方案治疗,对照组则给予常规治疗方法治疗。比较两组治疗前后血脂水平、炎症因子水平、Barthel生活指数以及预后复发状况。结果:两组经过治疗TC、TG、LDL-C的水平较治疗前均明显降低,HDL-C水平较治疗前均明显升高,但实验组治疗后的TC、TG、LDL-C与HDL-C水平较对照组治疗后变化更为明显,差异均具有统计学意义(P<0.05);两组治疗后Hs-CRP、TNF-ɑ、IL-8的水平较治疗前均明显降低,但实验组治疗后的Hs-CRP、TNF-ɑ、IL-8的水平较对照组治疗后降低更为明显,差异均具有统计学意义(P<0.05);经过治疗,两组的1个月评分、3个月评分与5个月评分均较治疗前明显提高,但实验组Barthel生活指数评分较对照组治疗后提高更为明显,差异均具有统计学意义(P<0.05);实验组治疗后复发率明显低于对照组,差异具有统计学意义(P<0.05)。结论:强化降脂治疗方案治疗急性期缺血性卒中患者,能够降低血脂水平和炎症因子水平,提高Barthel生活指数评分,减少急性期缺血性卒中患者复发率。

Objective：To explore effects of intensive lipid-lowering therapy on the prognosis of acute ischemic stroke in order to provide evidence for clinical research.Methods：A total of 82 cases of acute ischemic stroke were randomly divided into experimental and control groups with 40 cases and 42 cases in each group.The experimental group was treated with intensive treatment regimen,patients in the control group were treated with conventional therapy treatment.Changes in lipid levels before and after treatment were observed;changes in inflammatory factors between the two groups of patients before and after treatment.Barthel index of living conditions and prognosis of recurrence in patients before and after treatment were compared.Results：After treatment,TC,TG,LDL-C and HDL-C levels in both groups were decreased,but HDL-C was significantly increased（P〈0.05）.These changes were more obvious in the experiment group showing significant difference between the two groups.After treatment,Hs-CRP,TNF-ɑ,and IL-8levels decreased in both two groups,and the decrease was more obvious in the experiment group.After treatment,1-month,2-month,3-month and 5-months living Barthel Index scores were improved in both groups but the improvement was more obvious in the experiment group.The recurrence rate of the experiment group was significant lower than that of the control group（P〈0.05）.Conclusion：The intensive lipid-lowering therapy can significantly decrease blood lipid levels and inflammatory factors,improves life Barthel Index score and prognosis for patients with acute ischemic stroke.
