颅脑外伤患者脑血液中IL-6、BNP、CRP表达临床分析

目的:探讨血清脑钠肽(BNP)、白细胞介素-6(IL-6)、C反应蛋白(CRP)在老年颅脑损伤中的变化及临床意义。方法:采用放射免疫分析法及免疫比浊法测定80例的颅脑损伤及80例健康体检者血清BNP、IL-6及CRP水平。结果:颅脑损伤组血清IL-6和CRP水平均高于对照组,而BNP水平低于对照组,差异有统计学意义(P<0.05);与轻型及中型颅脑损伤患者相比,重型患者血清IL-6和CRP水平较高,而BNP水平较低,差异有统计学意义(P<0.05);与入院时相比,患者入院24h内血清BNP、IL-6和-CRP水平达到最高峰,在入院第3天后逐渐下降,与入院前相比,差异有统计学意义(P<0.05);死亡组血清IL-6和CRP水平高于存活组,而BNP水平低于存活组,差异均有统计学意义(P<0.05)。结论:血清BNP、IL-6和CRP水平测定将有助于评价老年颅脑损伤患者病变程度及预后。通过联合测定可提高上述指标在临床上应用的灵敏性及特异性。

Objective：To investigate changes in expression levels of brain natriuretic peptide（BNP）,interleukin-6（IL-6）,C-reactive protein（CRP）of patients with brain blood.Methods：The levels of IL-6,BNP,CRP of 80 cases of traumatic brain injury and 80 cases of healthy groups were determined with radioimmunoassay and immunoassays.Results：The levels of IL-6and CRP of patients with traumatic brain injury were significant higher than that of healthy group（P〈0.05）,while the levels of BNP of patients with traumatic brain injury were lower than healthy group（P〈0.05）.The levels of CRP and IL-6of patients with severe brain injury were higher than light and moderate traumatic brain injury patients（P〈0.05）.Compared with the time of admission,the serum BNP,IL-6and-CRP levels went to peak within 24 hour,then gradually decreased in 3dafter admission,showing significant difference compared with levels at admission（P〈0.05）.The levels of BNP of severe injured patients were lower than light and moderate traumatic brain injury patients（P〈0.05）.Conclusion：Determination of serum BNP,IL-6and CRP levels can help to prognose and evaluate severity of the disease in elderly patients with traumatic brain injury.
