正常婴儿髋关节超声多种评价参数的相关性研究 被引量:5

Relationship among parameters of ultrasound in normal infantile hip

摘要 [目的]研究正常婴儿髋关节超声多种评价参数的相关性及多种测量方法从不同角度评价髋关节发育的临床意义。[方法]对105例婴儿[男56例,女49例,汉族72例,回族33例,月龄1∽6个月,平均(2.6±1.3)个月]210个正常髋关节行超声检查,观察髋关节形态结构并测量不同参数(α角,β角,股骨头覆盖率,髋臼角,L值),比较不同参数相关性。[结果]1正常婴儿髋关节α角,β角,股骨头覆盖率测量值、髋臼角及L值测量值分别为(67.24±4.21)°,(62.92±6.40)°,(61.92±5.52)%,(23.04±2.56)°,(0.44±0.05);2正常男婴与女婴α角,β角,股骨头覆盖率,髋臼角及L值差异无统计学意义。回族与汉族婴儿正常髋关节α角,β角,股骨头覆盖率,髋臼角及L值差异无统计学意义;3α角与FHC呈正相关,与β角、髋臼角及L值呈负相关。[结论]Graf方法作为婴儿髋关节筛查的首选方法具有重要的临床意义;FHC、AI角、L值与α、β角具有相关性性,反映股骨头覆盖及骨性髋臼发育情况,对婴儿髋关节超声筛查具有借鉴意义。 [ Objective ] To study the ultrasonic scanning methods of normal infant hip and to determine whether there is a relationship among the multiple evaluation parameters. [ Methods] Two - hundred and ten hips of 105 infants were examined with ultrasound. The morphology and structure information of hips were observed, and multiple parameters ( angle a, angle β, femoral head percentage of cover, acetabular index, L parameter) were measured. [ Results] (1) The normal values of angle a, angle β, femoral head percentage of cover, aeetabular index, L parameter were (67.24 ± 4. 21 )°, ( 62. 92±6.40)°, (61.92±5.52)° , ( 23.04 ± 2. 56 )°, 0. 44 ± 0. 05, respeetively. (2)There was no statistical significance between boys and girls in angle a, angle [3, femoral head percentage of cover, acetabular index, L parameter. There were not statistical signifi- cant difference between the parameters of Hui nationality and the Han nationality normal infants. (3)For normal infant hips, angle a had positive correlation with FHC and had negative correlation with angle β, AI, L parameter. [ Conclusion ] Different measurement have different meanings . Graf' s method could be an early definite and screening method of choice in early diagno- sis of developmental dysplasia of the hip (DDH) . FHC, AI, L parameter could help the diagnose of DDH.
