李金斯坦疝修补术治疗高龄腹股沟疝患者在基层医院中的应用体会

Lichtenstein hernia repair for inguinal hernia in elderly patients: a experience of 84 cases in primary hospital

目的总结李金斯坦疝修补术（Lichtenstein）治疗高龄腹股沟疝患者的临床疗效和价值。方法回顾性分析2009年1月至2014年10月,新疆焉耆县人民医院收治的84例高龄腹股沟疝患者临床资料,采用Lichtenstein法,总结分析其手术时间、伤口疼痛、术后并发症及复发率。结果本组84例患者手术均顺利完成,手术时间48～72 min,平均（57±8）min,住院时间5～7 d,平均（6.0±1.0）d。术后出现切口感染5例,慢性疼痛3例,急性尿潴留8例,无补片感染。术后随访6～48个月,有1例复发。结论 Lichtenstein疝修补手术具有手术时间短、术后疼痛轻、复发率低、术后并发症少等优点,是治疗高龄腹股沟疝患者的有效手术方法,县级医院易于开展。

ObjectiveTo summarize the clinical efficacy and value of the Lichtenstein hernia repair in treatment of elderly patients with inguinal hernia. MethodsA total of 84 elderly patients with inguinal hernia admitted to Yanqi People′s Hospital from January 2009 to October 2014 were enrolled. The Lichtenstein repair was performed. The operation time, postoperative pain, complications and recurrence rate were analyzed. ResultsAll the cases were operated successfully. The operation time was 48 to 72 minutes, with a mean of （57±8） minutes. The hospital stay was 5 to 7 days, with a mean of （6±1） days. Wound infection was observed in 5 cases; 3 chronic pain and 8 acute urinary retention occur; no mesh infection was found. All patients were followed up for 6 to 48 months, and recurrences were observed in 1 of 84.ConclusionThe Lichtenstein hernia repair owns strengths of short-time, light pain, low recurrence rate and less complications. It is effective in treatment of elderly patients with inguinal hernia, and is easy to carry out in primary hospital.
