下颌无牙颌种植即刻修复的临床应用

Clinical application of fixed-detachable immediate restoration of dental implants with immediate loading in edentulous lower jaws

目的评价下颌无牙颌患者种植术后采用螺丝固定即刻修复体的临床应用效果。方法选择因重度牙周炎无法保留下颌余留牙患者11例,拔除余留牙后植入4-6枚种植体。术后48h内戴入种植支持式螺丝固位热凝树脂固定桥,行即刻功能性负载;4-6个月后永久固定修复。结果共58枚种植体中,56枚种植体初期扭矩达到35-45N·cm,行即刻修复,2枚种植体采用延期修复。在术后随访1-1.5年,种植体存活率达到100%。结论当植入种植体的终末扭矩达到35-45N·cm时,下颌无牙颌种植即刻修复可获得理想的中短期效果。

Objective To evaluate the clinical efficacy of detachable immediate screw-retained restoration in the patients with edentulous lower jaws. Methods Four to six implants were placed in each case of 11 patients with edentulous lower jaws, in whom the remaining teeth on the mandible could not be preserved because of severe periodontitis. Aresin bridge that was resin bridge laboratory-fabricated, screw-retained and heat-curing was delivered within 48 hours after surgeries and functional loading was applied immediately until the resin bridge was replaced by permanent prostheses 4-6 months later. Results Of 58 implants, the insertion torque reached 35-45 N · cm in 56 implants and restorations were performed immediately. The delayed restorations were performed in the rest 2 implants. Conclusion When the insertion torque of the implants reaches 35-45 N · cm, an ideal clinical short to mid-term efficacy can be obtained by fixed-detachable immediate restoration of dental implants with immediate loading in patients with edentulous, lower jaws.
