高危妊娠中胎盘炎症200例的妊娠结局分析

Analysis of pregnancy outcome of placenta infection in 200 cases of high risk pregnancy

目的 探讨高危妊娠中胎盘炎症的妊娠结局,研究胎盘炎症对妊娠造成的不良影响,改善产妇的妊娠结局。方法 将本院2013年12月~2014年12月收治的280例高危产妇分娩后的胎盘送病理学检查,将其中的80例正常胎盘组（送检无病理变化）作为对照组,另外200例胎盘发生炎症性病变组作为试验组,分析产妇的妊娠状态以及高危因素,探讨高危妊娠中胎盘炎症的妊娠结局,如新生儿窒息、死胎发生率之间的关系。结果 送检产妇中,正常胎盘占28.57%,发生胎盘炎症的占71.43%。试验组中,新生儿窒息24例（12.00%）、死胎发生率4.50%,均显著高于对照组的2.50%、1.25%,差异有统计学意义（P〈0.05）。结论产妇胎盘炎症可能会引起严重的高危妊娠,产妇应密切注意预防炎症,从而有效降低高危妊娠的发生率,改善产妇与新生儿的健康状况。

Objective To explore pregnancy outcome of placenta infection in high risk pregnancy, and research the adverse effects of placenta infection in pregnancy to improve puerpera＇s pregnancy outcome. Methods Placentas of 280 high-risk puerperas who were admitted to our hospital from December 2013 to December 2014 were send to do pathological examination. Normal placenta group （no pathological changes） with 80 eases were selected as control group, and the other 200 cases with placenta infection were selected as experimental group. Pregnancy status and high-risk factors of puerperas were analyzed, and pregnancy outcomes of placenta infection in high risk pregnancy were discussed, such as the relationship between neonatal asphyxia and the incidence of fetal death. Results Normal placenta accounted for 28.57%, and placenta infection accounted for 71.43% in pathological examination. In experimental group, there were 24（12.00%） cases of neonatal asphyxia, and the rate of fetal death was 4.50%, which were both significantly higher than 2.50% and 1.25% of control group. The difference was statistically significant （P 〈 0.05）. Conclusion Placenta infection in puerpera may cause serious high risk pregnancy. As a result, puerperal should pay close attention to prevention of inflammation, so as to effective reduce the incidence rate of high risk pregnancy and improve physical condition of puerpera and neonate.
